摘要
目的探讨异位胰腺的临床特点、诊断及外科处理原则。方法对我院近18年经手术切除和病理确诊的20例异位胰腺患者的资料进行回顾性分析。结果本组20例,男12例,女8例,年龄1-76岁,平均37.6岁。异位胰腺分布部位空肠7例,十二指肠4例,胃2例,胆总管2例,胆囊2例,纵膈1例,脾脏1例,后腹膜1例。术前仅1例确诊,7例误诊,12例漏诊。均行手术治疗,无一例出现术后并发症。结论异位胰腺的临床表现和检查手段无特异性,易漏诊和误诊,一旦发现,无论有无症状,均应早期手术治疗,以明确诊断及避免出现严重的并发症。
Objective To investigate the clinical features and management of ectopic pancreas in gastrointestinal tract. Methods 20 patients with ectopic pancreas confirmed by surgery and pathology in 18 years were retrospectively analyzed. Results The patients consist of 12 male and 8 female ,aged from 1 to 77 year old ,median age 37.6 years. 7 ectopic pancreases were found in the jejunum,4 cases in duodenum ,2 cases in stomach ,and 2 cases in common bile duct ,2 cases in gallbladder, 1 case in media sternum, 1 case in spleen and 1 case in abdomen cavity. Only one patient was diagnosed before operation. 7 patients were misdiagnosed and 12 cases＂ diagnosis was missed. All patients underwent surgery with nonevenfful recovery. Conclusions Ectopie pancreas is often lack of typical clinical manifestations. It is difficult to diagnose ectopic pancreas before operation. Misdiagnosis and missed diagnosis can happen easily. Once it is found, whenever the patient has symptoms or not, complete remove of ectopic pancreas is the most effective treatment to prevent the severe complications caused by ectopic pancreas.
