腕舟骨芯假体的受力分析研究 被引量:3

摘要 目的分析纳米羟基磷灰石/聚酰胺仿生材料舟骨芯假体腱球置换后,腕关节的生物力学性能。方法12具新鲜腕关节标本,分别经过舟骨近端切除、舟骨完全切除、舟骨近端单纯腱球置换、舟骨近端纳米羟基磷灰石/聚酰胺仿生材料舟骨芯假体腱球置换、舟骨单纯腱球置换、舟骨纳米羟基磷灰石/聚酰胺仿生材料舟骨芯假体腱球置换等7种处理,在50、100、150、200、250N的外力作用下,测量桡腕关节和尺腕关节的应力分布和受力面积。结果舟骨近端切除,舟骨完全切除后腕关节应力分布和受力面积发生明显改变(P<0.05);单纯腱球置换后,桡腕关节应力减小,受力面积增大(P<0.05);纳米羟基磷灰石/聚酰胺仿生材料舟骨芯假体腱球置换后,桡腕关节应力和受力面积同正常情况相比较无明显改变(P>0.05)。结论舟骨近端切除和完全切除,单纯腱球置换后,腕关节的应力分布和受力面积发生明显改变;纳米羟基磷灰石/聚酰胺仿生材料舟骨芯假体腱球置换后,腕关节的应力分布和受力面积同正常相接近。 Objective To study the biomechanical properties of the carpal after scaphoid excision and the tendon ball replacement with nano- hydroxyapatite/polyamide(n-HA/PA66) biomaterial scaphoid core prosthesis. Methods Twelve fresh cadaveric wrists were treated bY the proximal of scaphoid excision,the scaphoid excision, the unitary tendon ball replacement of scaphoid and the tendon ball replacement of the scaphoid with n- H A/PA66 biomaterial scaphoid core prosthesis. 50 N, 100 N, 150 N, 200 N, 250 N loads were applied to the cadaveric wrists. The stress distribution and the contact area of the radialcarpal and unlarearpal joint were measured. Results The contact area and the stress distribution of the wrist were singnificantly change after the proximal of scaphoid excision and the seaphoid excision(P〈0.05). After the unitary tendon ball replacement,the contact area of the radialcarpal joint was singnificantly increased,while the stress distribution of the radialcarpal joint was singnifieantly decreased(P〈0.05). After the the tendon ball replacement with n-HA/PA66 biomaterial scaphoid core prosthesis,the contact area and the stress distribution of the radialcarpal joint didn't changed singnificantly(P〉0. 05). Conclusion The contact area and the stress distribution of the wrist changed singnificantly after the proximal of scaphoid excision,the scaphoid excision and the unitary tendon ball replacement. While the contact area and the stress distribution of the wrist do not change singnifieantly after the tendon ball replacement with n-HA/ PA66 biomaterial seaphoid core prosthesis.
