2型糖尿病胰岛素抵抗与TNF—α、肥胖、脂代谢异常的关系

摘要 目的探讨2型糖尿病患者胰岛素抵抗与TNF-α、肥胖、脂代谢异常紊乱的关系。方法测定109名2型糖尿病患者(糖尿病组)及86名健康对照者(对照组)身高、体重、腰围、腹围、臀围及空腹血清TNF-α、胰岛素、血糖、血脂水平,计算所有受试者胰岛素抵抗指数,并行LogiStic回归分析。结果(1)肥胖者血清TNF-α水平均明显高于非肥胖者(P〈0.01)。(2)糖尿病组血清TNF-α水平、体脂参数均明显高于对照组(P〈0.05)。(3)经Logistic分析发现胰岛素抵抗指数与TNF-α、腰/臀比、甘油三脂水平密切相关。结论(1)腰/臀比是影响胰岛素抵抗的主要因素,其对胰岛素抵抗的影响部分是通过TNF—α、甘油三脂介导的。(2)2型糖尿病患者血清TNF-α水平明显升高。 Objective To study the ,relationship between insulin resistant and TNF- α obesity, dyslipidaemia in patients with type 2 diabetes mellitus. Method 109 patients with type 2 diabetes and 86 normal controls were studied. Length, weight,waist, abdomen, hip,plasma TNF- α plasma lipids, fasting insulin, fasting plasma glucose, were measured. Homeostasis model assessment(HOMA) was applied to estimate the degree of insulin resistant. Various factors analysis was conducted with logistic regression analysis. Result (1) Plasma TNF- α levels in obese subjects were much higher than those in normal weight subjects. (2)TNF-α levels and body adipose parameters in diabetic group were much higher than controls. (3) Logistic analysis showed that waist-hip ratio, TNF- α, triglyceride were risk factors for insulin resistant. Conclusion (1) Waist-hip ratio was the most important risk factors for insulin resistant, lts effect was conducted by TNF- α and triglyceride. (2) There was an increase of TNF- α in type 2 diabetes.
