摘要
目的探讨多层螺旋CT检查对闭合性肠及肠系膜损伤的诊断价值。方法回顾性分析30例经手术证实的闭合性肠及肠系膜损伤患者的CT表现,通过对比分析,总结各种征象的临床意义。结果12例十二指肠损伤,10例空回肠及系膜损伤,4例结肠及系膜损伤,4例多部位损伤。CT征象为:腹腔及腹膜后积液25例,肠管壁增厚21例,肠系膜浸润11例,肠系膜血肿6例,肠壁血肿5例,腔外游离气体6例。结论多层螺旋CT扫描是早期诊断急性闭合性肠及肠系膜损伤的重要方法,具有高度的敏感性及特异性。
Objective To evaluate the value of multi-slice spiral computed tomography （MSCT） in the diagnosis of bowel and mesenteric closed injury. Methods To retrospectively analyze the MSCT features of 30 patients with bowel and mesenteric closed injury confirmed by surgical operation. Based on the contrastive analysis,the clinical significanee of these features were summed up. Results Twelve cases had injury in duodenum,10 in jejunoileum and mesenterium,4 in colon and mesenterium,4 cases with combined injury. The CT features included intra-abdominal and retroperitoneal free fluid （25 cases）,bowel-wall thickening （21 cases）, mesenteric infiltration （11 cases）,mesenteric hematoma （6 cases）, intestinal wall hematoma （S cases）, pneumoperitoneum （6 cases）. Conclusion MSCT is the most important method for early diagnosis of acute bowel and mesenteric closed injury and has high sensitivity and specificity.
