摘要
目的回顾性评价经肝动脉节段化疗栓塞治疗原发性肝癌的安全性、技术成功率及临床疗效,以期提高患者生活质量及生存率。方法本院近3年32例原发性肝癌实施节段性化疗栓塞治疗作为研究组,随机抽取前期20例非节段化疗栓塞治疗作为对照组,比较两组患者的肿瘤大小、肿瘤血管的改变、生存率等差别。结果研究组较对照组术后肿瘤缩小更明显(χ2=7.61,P<0.01),甲胎蛋白下降显著(χ2=7.803,P<0.05)。研究组6、12、24、36个月生存率分别为93.8%(30/32)、68.8%(22/32)、34.4%(11/32)、18.8%(6/32),对照组6、12、24、36个月生存率分别为60%(12/20)、40%(8/20)、20%(4/20)、5%(1/20),两组生存率差异有统计学意义(P<0.01)。结论肝动脉节段化疗栓塞治疗原发性肝癌安全、疗效明显,其疗效明显优于非节段化疗栓塞治疗方法。
Objective To retrospectively evaluate the safety and technical successful rates and curative effect of transcatheter segamental hepatic arterial chemoembolization（TACE） for hepatocellular cancer and to improve the life quality of patients and to extend survival time. Methods The 32 cases HCC for transcatheter segamental arterial chemoembolization（TACE） were study group in our hospital recently 3 years,20 cases HCC with transcatheter non segamental arterial chemoembolization（TACE） were compare group,to compared parameters of hepatic tumors size and change of arterial vessel for cancer and survival rate et al in two groups. Results There was significant difference between the two groups in tumor, s size （χ^2 = 7. 61, P〈0.01）. There was significant difference between the two groups in AFP change （χ^2 = 7. 803, P〈 0. 05）. The survival rate of study group respectively at 6 months,1 year,2 years,3 years was 93.8% （30/32）, 68.8% （22/32）, 34. 4% （11/32）, 18.8% （6/32）and the compare group was 60 % （12/20）, 40 % （8/20）, 20 % （4/20）, 5 % （1/20） respectively. There was significant difference between the two groups in survival rate（P〈0.01）. Conclusion Transcatheter segamental hepatic arterial TACE for HCC is safety and very effectively. It＇s curative effect is much better than non segamental arterial TACE for HCC.
