摘要
目的:应用 Amplatzer 血管塞封堵动脉导管未闭(PDA)并对其疗效进行评价。方法:全组共11例,年龄4.0～17.0(10.09±5.17)岁,体重15～63(35.05±16.20)kg。PDA 长度为8.4～20.9(14.09±3.50)mm,最狭窄处均位于肺动脉侧,直径为1.0～2.0(1.48±0.35)mm;经导管置入 Amplatzer 血管塞封堵PDA。封堵后5～10 min 行胸主动脉造影评价疗效,术后24 h、1个月、3个月及1年进行随访观察有无残余分流及再通。结果:全组技术成功率为100%,无任何并发症。术后10 min 胸主动脉造影示11例动脉导管均完全闭合,无残余分流;5例完成1年随访,PDA 完全闭合,未见再通。结论:应用 Amplatzer 血管寨封堵类似于长管状的 PDA 是一种安全、有效的非手术方法,技术成功率高,即刻效果好,近、中远期随访疗效可靠。
Objective ：To conduct transcatheter closure of patent ductus arteriosus （PDA） by Amplatzer vascular plug and to preliminarily evaluate the safety and effect of the treatment. Methods ： A total of 11 PDA patients with the mean age of 10. 09 ± 5. 17 years （ ranging from 4. 0 to 17.0 years） and the mean body weight of 35.05 ± 16.20 kg（ ranging from 15 to 63 kg）were occluded by Amplatzer vascular plug. The length of PDA were 8.4 - 20. 9 （ 14. 09 ± 3. 50） mm, the most narrow spots were at the side of pulmonary artery with the diameter of 1.0 - 2.0 （ 1.48 ± 0. 35 ） mm. Aortography was performed to evaluate the effect of 5-10 minutes after the procedure. Echocardiography was performed at 24 hours,one month,three months and 1 year after the closure to examine whether there was residual shunt and recanalization. Results：The technical successful rate was 100% without complications. Aortography revealed there were complete occlusion without residual shunt in all 1 lcases at 10 minutes after the procedure. One year follow up was done in 5 patients and no residual shunt observed. Conclusion ：Transcatheter closure of PDA with Amplatzer vascular plug was a safe and effective method ,it has the high successful rate with the complete closure immediately after the procedure ;the short and mid-term follow-up showed a good result as well.
