原发性高血压患者红细胞免疫状态及其与循环免疫复合物的关系

摘要 目的探讨原发性高血压(EH)患者红细胞免疫状态及其与循环免疫复合物(CIC)的关系。方法用酵母菌法和光电比色法检测102例EH患者红细胞免疫粘附能力、红细胞免疫粘附调节因子及CIC。结果ⅡⅢ期EH患者红细胞C3b受体花环率(RBC—C3bRR)低于正常人,且ⅠⅡⅢ期呈进行性降低。红细胞免疫复合物花环率(RBC—ICR)与正常人无显著性差异,红细胞免疫粘附促进因子活性低于正常组,且与RBC—C3bRR有正相关关系,红细胞免疫粘附抑制因子变化不显著,Ⅰ期EH患者CIC正常,而ⅡⅢ期EH患者CIC的阳性率显著高于正常人。结论EH患者存在继发性的红细胞免疫粘附能力下降和CIC升高,此改变可能与红细胞膜上的C3受体数目减少和继发性的血管壁组织损伤有关。 Objective To study red-cell immune state and circulation immune complex (CIC) in patiemts with essential hypertention (EH). Methods The red-cell immune adherence (RCIA), red-cell immune absorption regulatory factor (RCIARF) and CIC were detected with Guel's method and colorimetric analysis in 102 patients with EH. Results The rosette rate of red blood cell C3b receptor (RBC-C3bRR), which was progressively in patients with EH stage Ⅰ, stagy Ⅱ and stagy Ⅲ, was lower than that in normal group. There was no significant difference in normal group. There was no significant difference in rosette rate of RBC immune cemplex(RBC-ICR) between normal group and patients with EH. The activity of RCIARF was lower than that in normal group. There was a positive correlation between RCIARF and RBC-C3bRR. There was no remarkable change in inhibitory factor. CIC was normal in patients with EH stage I while positive rate of CIC in patients with EH stage Ⅱ and stage Ⅲ was remarkably higher than that in normal group. Conclusion In patients with EH, the RCIA decreases and CIC increases, which may be related to the reduce of C3 receptor type I on the erythrocyte and the secondary demages to vascular tissure.
