摘要
目的探讨基质金属蛋白酶9(matrix metalloproteinase 9,MMP-9)、上皮型钙黏附蛋白(E-cadherin,E-cad)在原发性干燥综合征(primary Sjgren syndrome,PSS)发病过程中的作用及意义。方法采用免疫组化链霉素抗生物素-过氧化酶(SP)法检测52例PSS患者、30例正常对照组唇腺中MMP-9,E-cad表达及分布情况。结果52例PSS患者唇腺组织中MMP-9均阳性高表达,导管区平均表达水平,面积为(2 135.50±566.75)μm2,光密度为(0.211±0.068)A/μm2,累积光密度为(460.26±357.17)A,高于正常对照组(均P<0.01),腺泡区平均表达水平,面积为(19 981.15±3551.38)μm2,光密度为(0.167±0.027)A/μm2,累积光密度为(3 320.53±1 132.99)A,低于正常对照组(均P<0.01),MMP-9表达随着淋巴细胞浸润灶数的增加而增强。52例PSS组E-cad均低表达,导管区平均表达水平,面积为(2 493.05±997.14)μm2,光密度为(0.209±0.007)A/μm2,累积光密度为(489.33±248.21)A,低于正常对照组(P<0.05或P<0.01),腺泡区平均表达水平,面积为(1 1957.66±6 975.28)μm2,光密度为(0.108±0.003)A/μm2,累积光密度为(1 291.42±951.79)A,低于正常对照组(P<0.01),E-cad表达随着淋巴细胞浸润灶数的增加而降低。PSS患者唇腺组织中MMP-9与E-cad的表达呈负相关(P<0.01)。结论MMP-9、E-cad在PSS患者唇腺组织中存在异常表达,二者的表达情况与病理分级相关,MMP-9、E-cad参与了原发性干燥综合征的发病过程。
Objective To study the expression of matrix metalloproteinase-9(MMP-9),E-eadherin(E-cad) in the labial gland of the patients with primary Sjogren syndrome(PSS) for its role in the pathogenesis. Methods Biopsies of labial glands were obtained from 52 patients with PSS and 30 healthy controls. The immunohistochemieal staining was used to detect the expression of MMP-9, E-cad. Quantitative analysis was performed by image analysis software. Results The over-expression of MMP-9 in 52 PSS patients,the mean over-expression level of MMP-9 in the ductal region area (2 135. 50± 566.75) um^2 , optical density (0. 211 ± 0. 068) A/um^2 , integrated optical density (460. 26 ±357.17) A in PSS patients were significantly higher than those in healthy controls (all P 〈 0.01). The mean overexpression level of MMP-9 in the acinic region area (19 981.15 5= 3 551.38) um^2 , optical density (0. 167 5= 0. 027) A/um^2 ,integrated optical density (3 320.53 ± 1 132.99) A in PSS patients were significantly higher than those in healthy controls(all P 〈0.01). There was a positive correlation between lymphocyte focus score and expression of MMP-9. The reduce-expression of E-cad in 52 PSS patients, the mean reduced-expression level of E-cad in ductal region area (2 493. 05±997.14) um^2 , optical density (0. 209 ± 0. 007) A/um^2 , integrated optical density (489.33±248.21) A, in PSS patients were significantly lower than those in healthy controls ( P 〈0.01, P 〈0.05), the mean reducedexpression level of E-cad in the acinic region area (11 957.66 ± 6 975.28) um^2 , optical density (0. 108 ± 0. 003) A/um^2 , integrated optical density (1 291.42 ± 951.79) A in PSS patients were significantly lower than those in healthy controls. There was a negative correlation between lymphocyte focus score and expression of E-cad. There was linear negative correlation between expression of MMP-9 and E-cad in PSS patients (P 〈 0.01). Conclusion The expression of MMP-9 and E-cad are abnormal in the labial gland of the patients with PSS,the expressed degree is correlated with pathological grade, the interaction of MMP-9 ,E-cad may have an important role in the pathogeny of PSS.
