摘要
目的:探讨关节镜下前交叉韧带损伤重建术后的功能康复。方法:回顾分析了1990年~1997年间我科收治并且目前能随访到的21例膝前交叉韧带损伤后采用关节镜下双股半腱肌腱重建的患者,术后均采用我科制订的系统康复治疗计划进行康复训练,随访时间98~168个月,平均127.36个月。对其术前和近期的自我感觉、查体及Lysholm膝关节评分、IKDC(International Knee Documentation Comminee)分级标准评定的情况进行分析。结果:21例术前弹响、交锁和打软腿的症状均消失,浮髌试验均为阴性,均无过伸痛。关节轻度疼痛11例,Lachman征阳性3例,前抽屉试验阳性8例,但均无关节不稳的主诉。无取腱造成的其它并发症。Lysholm膝关节评分术后优于术前,差异有显著性意义(P<0.05)。IKDC评分结果术后明显优于术前。术后平均4.65个月恢复日常活动,恢复体育锻炼时间均在6个月左右。有2例伸直受限(差10°),所有患者屈曲活动度基本正常,结论:ACL损伤重建的术后患者经过我们所设计实施的康复计划的训练后,没有关节不稳的主诉,屈伸活动度满足日常生活需要,对术后的功能均较满意。
Objective:To explore the rehabilitation training for postoperative anterior cruciate ligament(ACL)reconstruction under arthroscopy.Methods:21 retrospective cases who underwent surgical treatment of arthroscopic ACL reconstruction with double-bundle semitendinosus during 1990~1997 were analyzed.Our rehabilitation training plan was performed postoperatively.The mean follow-up time was 127.36 months(98~168 months).The analysis was performed on the symptoms,physical signs,Lysholm and IKDC scores before operation and recently.Results All the patients had good clinical results with no click,locking,"give way",extra-extension pain and floating patella test were negative.There were no other complications correlated with semitendinosus grafting.But mild pain were found in 11 cases,Lachman test were positive in 3 cases,anterior drawer test were positive in 8 cases without complications of instability. There were differences in Lysholm scale between preoperation and postoperation(P〈0.05).The IKDC evaluation of postoperation was improved significantly than preoperation.The average time of healing was 4.65 months after operation and the patients begun to do the physical training after 6 months.There were 2 cases that had not full extension(-10°).All the patients' flections were normal.Conclusions:With the rehabilitation training of ours,all the patients had good clinical results and satisfaction.
