摘要
目的了解南华大学附属第一医院大肠埃希菌对抗菌药物的耐药情况，指导临床合理用药。方法应用API半自动微生物鉴定系统和用K-B法进行细菌鉴定及药物敏感性测定，参照1999年NCCLS的标准进行结果判断。结果2006年1月～2007年12月共检测出大肠埃希菌422株，产超广谱13内酰胺酶（ESBk）的大肠埃希菌占33．4％，2007年比2006年有所增加，产ESBLs大肠埃希菌对青霉素类和头孢菌素类抗菌药物大多耐药，对亚胺培南和美洛培南敏感，对其它类抗菌药物耐药率也较不产ESBLs大肠埃希菌高。结论产ESBLs大肠埃希菌耐药率高，且有增加趋势，临床上应参照药敏结果，合理使用抗菌药物。
Objective To know the drug resistance of Escherichia coli（ E. coli） in First Affilicated Hospital of Nanhua University, and direct clinical rational use of drug. Methods Bacterium accredit and drug sensitive were determined by API semi-automatic microorganism accredit system in K- B method. The result was judged by NCLLS of 1999. Results From 2006 to 2007 a total of 422 E . coli were detected , 33.4% of it was the ESBLs （ Extended spectrum β lactamases） and in 2007 it has increased compored with 2006. The Escherichin coli with ESBLs were almost resistant to penicillin and cephalosporins and senstive to lmipenem and Meropenemt and to other antibacterial drag,which resistance rate is higher than that did not produce ESBLs. Conclusions Resistance of Escherichia coli that produce ESBLs is higher than it not produce, and it will increase in future, susceptibility test results should be referenced for rational use of drug in clinic.
