摘要
目的探讨后路脊柱手术后感觉异常性股痛（meralgia paresthetica，MP）发生的病因学基础和临床相关因素分析。方法回顾（1998．1-2007．6）近10年间收治的3159例后路脊柱手术患者，采集患者的性别、年龄、身高、体重、手术时间、术中是否使用凝胶衬垫等相关资料，对术后MP的发生、严重程度及恢复情况进行统计，分析诸因素与MP发生之间的相关性，并将相关显著的变量引入多因素非条件logistic回归模型进行综合分析，评判其相对危险性。资料采集中排除糖尿病性末梢神经炎、腰脊神经本身疾患等所致的MP。结果3159例脊柱后路手术患者中402例术后发生MP，总发生率为12．7％。术中使用凝胶衬垫1272例，65例发生MP，发生率为5．11％；术中未使用凝胶衬垫1887例，337例发生MP，发生率为17．86％。术中凝胶衬垫的使用、患者身高、年龄和手术时间与术后MP的发生有明确相关性（P〈0．05），相关程度依次升高，0月值分别为0．029、0．831、1．060和36636．286，其中手术时间的影响最大，并远远高于其他诸因素。结论MP是脊柱后路手术术后并发症之一，重视发病因素的影响并合理改善手术条件是减少MP发生的有效措施，其中缩短手术时间最为关键。
Objective To explore the etiological basis and risk factors of meralgia paresthetica after posterior spine surgery. Methods From January 1998 to June 2007, 3,159 patients after posterior spine surgery were retrospectively analysed in our hospital. The data included the patient＇s gender, age, height, weight, surgery time, and whether using resinic pad during operation. To observe the occurrence, severity and recovery of meralgia paresthetica （MP）, analyse the correlation between the informations and MP, finally introduce the significant related variables into a non-Logistic regression model, evaluate their risk. All patients were excluded the MP due to diabetic peripheral neuritis and abnormal sensation in the lateral thigh preoperatively caused by the lumbar spinal nerve disorder itself. Results There were 402/3159 patients suffered from meralgia paresthetica after posterior spine surgery （12.7%）. In the using resinic pad group, there were 65/1272 patients suffered from MP, and the incidence rate was 5.11%; while in the without resinic pad group, there were 337/1887 patients suffered from MP, and the incidence rate was 17.86%. Multinomial unconditional logistic regression indicated that the resinic pad using, the patient height, age and operating time had a clear correlation with the occurence of MP （P〈 0.05）, and the correlativity was stepped up in order, their odds ratios value was 0.029, 0.831, 1.060 and 36636.286, the operative time was the greatest impact, and far higher than all other factors. There was no significant effect of weight to the occurence of MP （P= 0.078）. Conclusion Meralgia paresthetica is one of the complications of posterior spine surgery. Thinking highly of the risk factors and improving operative conditions is the important and effective measurements for reducing it, and to shorten the operation time is the most crucial.
