摘要
目的:探讨囊袋张力环植入在治疗晶状体半脱位手术中的安全性及预防并发症的疗效。方法:对21例21眼晶状体半脱位患者(脱位范围45°~120°),13眼(13例)行白内障超声乳化+囊袋张力环+折叠式人工晶状体植入术,8眼(8例)行小切口白内障囊外摘除+囊袋张力环+折叠式人工晶状体植入术。结果:21例21眼晶状体半脱位的白内障患者均顺利植入了囊袋张力环及人工晶状体于囊袋内,术后人工晶状体正位20例(20眼),人工晶状体轻度倾斜1例(1眼),后囊膜平整无皱褶19例(19眼),后囊膜皱褶2例(2眼),眼压升高2例(2眼),观察3个月均无后囊膜混浊。结论:对于晶状体半脱位患者,囊袋张力环的应用可增加手术安全性,维持晶状体囊袋的完整性、人工晶状体的正常位置,抑制囊袋皱缩及降低后发性白内障的发生率。
Objective To investigate the safety and efficacy of capsular tension ring implantation in the surgical treatment for subluxated lens. Methods 13 of 21 patients with lens subluxation (ranging from 45° - 120°) received phacoemulsification plus capsular tension ring and fold intraocular lens implantation, the remaining 8 received extracapsular cataract extraction plus the same implantation. Results Both capsular tension ring and artificial lens were successfully implanted into the capsule in all the patients. After surgery, 20 patients had an intraocular lens on the right position and only one lens was slightly tilting. 19 patients had smooth posterior capsular membrane without shrinkage but 2 had slight shrinkage. Intraocular pressure was elevated in two patients. No turbid posterior capsular membrane occurred during a 3-month follow-up. Conclusions The application of capsular tension ring in lens subluxation could improve the safety of surgery, maintain the integrality of the lens capsule and the fight position of the artificial lens, inhibit the shrinkage of the capsule, and decrease the incidence of secondary cataract.
