摘要
目的探讨蝶窦炎性病变的诊断与手术治疗。方法回顾性分析1996年5月～2006年12月的25例蝶窦炎性疾病的临床表现、影像学特点、手术治疗结果和随访。结果本病主要症状为头痛和眼部症状,25例患者均经CT检查,均在全身麻醉经鼻内镜下蝶窦开放手术,术后病理确诊,症状先后缓解或消失,随访6个月至2年末见复发。结论蝶窦炎性疾病临床表现无特异性,易误诊;鼻内镜和CT检查能提高诊断率,鼻内镜下蝶窦手术是治疗蝶窦炎性疾病安全、有效。
Objective To explore better diagnostic program and surgical therapeutic procedures for inflammatory sphenoid sinus lesions based on a clinical study. Methods A retrospective study was carried out among 25 cases with inflammatory sphenoid sinus lesions treated in our Hospital from May, 1 996 to Dec, 2006. Their clinical data, CT imaging features, outcome of surgical therapy and the results of following up were carefully analyzed in order to sum up the desired procedures for the diagnosis and the experience with the surgical therapy for such a kind of lesions. Results The main clinical manifestation among these studied cases included headache and ocular symptoms, and they were all treated by sphenoid sinus opening operaion under nasal endoscope with general anesthesia,with their mucosal samples taken from the sinus during the surgery for pathohistological assaying. By tlle end of following up period 1asted for 6 months to 2 Years, the main symptoms,like headache and bloody postnasal drip, were disappeared among 23 cases, and headache was remission in 2 cases. Among the 5 patients with hypopsla, 4 were improved in their visual acuity. No case was seen with the lesion reoccurred during the following up period. Conclusions There may be no specific clinical manifestation for inflammatory sphenoid sinus lesions so as to cause a misdiagnosis easily in clinic for such a kind of condition. When nasal endoscopy and CT scanning procedures were used in combination for this kind of lesions, the diagnostic efficacy may be improved largely. Furthermore, the therapeutic procedures can be carried out more safely and effectively with sphenoid sinus operation performed under nasal endoscope for the lesion.
