摘要
目的:通过对冠状动脉造影正常的糖尿病与非糖尿病患者的校正TIMI帧数(CTFT)的对比分析,评估冠状动脉心肌水平的灌注情况与2型糖尿病的关系。方法:冠状动脉造影正常的126例患者,依据有无糖尿病分为两组:2型糖尿病组55例,非糖尿病组71例。通过冠脉影像系统,分析两组的右冠状动脉(RCA)、前降支(LAD)以及回旋支(LCX)的CTFT。结果:2型糖尿病组的RCA、LAD及LCX的CTFT分别为:(16.92±7.95)帧、(17.33±9.06)帧、(20.84±6.98)帧,均长于非糖尿病组的:(21.53±8.64)帧、(23.70±8.27)帧、(22.15±7.51)帧,其中RCA及LAD间的差异有显著性(P<0.05,P<0.01),而回旋支的CTFT差异无显著性。结论:冠状动脉造影正常的2型糖尿病患者在心肌灌注水平上与非糖尿病者有区别,其机制可能与糖尿病患者的冠脉内皮细胞功能紊乱及其所导致的微血管病变相关。
Objective:To investigate the comparison of corrected TIMI frame count(CTFC) in type 2 diabetes mellitus patients with normal coronary angiography and patients without type-2 diabetes melhtus,and evaluate the relationship between myocardial perfusion of coronary angiography and type 2 diabetes melhtus.Methods: 126 patients with normal coronary angiography were divided into 2 groups according to the diagnosis of diabetes mellitus: 55 patients with type 2 diabetes mellitus and 71 patients without diabetes mellitus.The CTFT of RCA, LAD, LCX had been analysed in both groups through image analysis system.Results : The CTFT of RCA, LAD, LCX in diabetes group was 16.92±7.95 frames,17.33±9.06 frames,20.84±6.98 frames respecitvely,and the CTFT in nondiabetes group was 21.53± 8.64 frames,23.70±8.27 frames,22.15±7.51 frames respectively,the CTFT of the former was greater than that of the latter. There were statistical differences in the CTFT of RCA and LAD,and there was no statistical difference in the LCX between both groups.Conclusion: On the level of myocardial perfusion,there was distinct difference in patients with type 2 diabetes mellitus and the nondiabetes group.The mechanism may be related to the endothelial cell dysfunction and miereangiopathy in patients with diabetes mellitus.
