摘要
介绍以旋髂浅动脉为主要轴型血管的髂腹部皮瓣移植修复手部创伤或瘢痕畸形所致软组织缺损共21例;有三例同时修剪多余的皮下脂肪,应用显微外科技术保留供血血管,克服该皮瓣臃肿的缺点。
Since 1984 through 1988, 21 patients with soft tissue defects of hand were re-. paired with pedicle iliac abdominal flap grafts carrying superficial circumflex iliac artery. Patients' age ranged from 2-46 years old, 15 males and 6 females. Twelve cases were burn scar contractures, 3 electrical burns, 6 crush injuries. All defects were associated with exposed and/or damaged vessels, nerves. tendons, bones or joints. The excessive subcutneous fat was removed and subdermal vessels preserved under microscope to form thin flaps in three cases. All flaps were survived. The function and appearance of the reconstructed hands were satisfactory. Postoperative mobilization and exercise of joints can be achieved early; there were effective covering for various defects, the donor site can be closed primarily in most cases at inconspicuous location.
