摘要
目的分析后腹腔镜手术中转开放手术的原因。方法对195例后腹腔镜手术中17例转开放手术的原因及过程进行回顾性分析。结果17例术中转开放手术,包括肾上肿瘤切除6例,肾囊肿4例,肾切除4例,肾癌根治性切除3例。结论严格掌握后腹腔镜手术适应证,熟练后腹腔镜操作技巧,是提高后腹腔镜手术成功率的关键。
Objective To analyze the causes of retroperitoneal laparoscopy converted to open surgery in urology. Methods The study included 195 patients undergoing retroperitoheal laparoscopy surgery in our department.A total of 17 （8.7%）patients converted to open surgery during the procedure. Results The operations were successfully accomplished in 168 cases,17 cases converted to open operation, including 6 patients in adrenalectomy ,4 multiple renal cyst, 4 nephrectomy,3 radical nephrectomy.Different retroperitoneal laparoscopy procedures had different proportion of conversion to open surgery. In retroperitoneal laparoscopy adrenalectomy 13.0%（6/46） of patients converted to open surgery, 4.4%in renal cyst excision （4/90）, 11.1% in nephrectomy（4/36）,13.0% in radical nephrectomy（3/23）. Conclusion Selection of proper indication,lmprovement of skill is essential for retroperitoneal laparoscopy.
