258例冠心病患者窦性心率震荡的观察 被引量:1

Study of the Change and Significance of Heart Rate Turbulence in 258 Patients with Coronary Artery Disease

摘要 目的探讨冠心病伴室性早搏患者窦性心率震荡(HRT)的变化规律及临床价值。方法应用24小时动态心电对258例伴有室性早搏的冠心病患者和186例无器质性心脏病的室性早搏患者进行窦性心率震荡检测分析比较,并计算震荡初始(TO)和震荡斜率(TS)与左室射血分数(LVEF)、心率变异性(SDNN)之间的相关性;对冠心病心肌梗塞组与非心肌梗塞组、心肌梗塞组有无不良心血管事件进行比较,计算其震荡初始(TO)和震荡斜率(TS)。结果冠心病组TO明显高于对照组,TS明显低于对照组(P<0.05);死亡组TS明显低于生存组(P<0.05);TOTS与左室射血分数(LVEF)、心率变异性(SDNN)无明显相关(P>0.05)。结论冠心病患者窦性心率震荡现象减弱,故窦性心率震荡可用于判断冠心病患者预后情况。 Objective To study the change and significance of heart rate turbulence in patients with coronary artery disease (CAD). Methods Application of a 24-hour Dangtaixindian of 258 cases of patients with coronary heart disease with premature ventricular contractions and 186 cases of non-organic heart disease patients with sinus Analysis of heart rate shocks, and calculate the initial shocks (TO) and shocks slope (TS) and left veutricular ejection fraction (LVEF) and heart rate variability (SDNN)the correlation between;group of coronary heart disease and myocardial infarction group of non - myocardial infarction, myocardial infarction group whether adverse cardiovascular events compared Calculate the initial shock (TO) and shocks slope (TS). Results Group TO significantly higher than that of coronary heart disease, TS was significantly lower than the control group ( P 〈 0.05 ) ; have adverse cardiovascular events group TO high, TS low, the differences were significant (P 〈0.05 ) ; TO TS And left ventricular ejection fraction (LVEF), heart rate variability (SDNN) no significant correlation (P 〉 0. 05 ). Conclusion Sinus patients with coronary heart disease is weakened heart rate shocks, the sinus heart rate shocks can be used to determine the prognosis of patients with coronary heart disease cases.
