摘要
目的探讨腹腔镜急诊胆囊切除术在基层医院的应用、处理方法及并发症的防治。方法我们自2002年10月至2008年2月共行腹腔镜下胆囊切除术641例；回顾分析其中急性发作的胆囊炎胆囊结石病人305例（4758％）临床资料及治疗效果。结果本组305例，腹腔镜手术痊愈280例（9180％），中转开腹手术25例痊愈（820％）。术后ERCP取石5例（1.64％），第二次手术后痊愈4例（0.98％），死亡1例（0.33％）。结论腹腔镜急诊胆囊切除术所遇情况复杂、多变，需严格掌握腹腔镜手术技术熟练程度及急诊手术适应症，做好急诊术前准备，术中正确及时掌握中转开腹手术时机。腹腔镜急诊胆囊切除术适宜在基层医院的应用。基层医院要适应变化，积极主动地开展此项工作。
