人体腧穴组织形态特异性研究的进展与现状 被引量:29

摘要 对腧穴组织形态的研究显示,与非穴区比较,腧穴具有表皮薄、神经末梢丰富、感受器密集、血管和淋巴丰富等特点,除神经外,肌梭、结缔组织也可能是穴位效应特异性的结构基础,近年来对穴位形态的三维立体结构研究得到重视。穴位通常被认为具有高导电量、低阻抗、高电容、高电压等明显的电特性,研究穴位电特性的新技术新方法也不断出现。穴区高Ca2+,低Na+离子等生物化学特性多有报道。许多基础及临床研究显示出针刺穴位相对于非穴、他穴,其治疗效应具有特异性。运用脑功能成像技术研究穴位针刺的结果表明,人脑对不同穴位刺激具有特异的应答,该技术有望成为研究穴位特异性的新手段。有部分研究未得出支持腧穴具有特异性的证据。腧穴特异性研究结果的不一致,说明仍需进行方法学上的探索;腧穴相对特异性的内涵、研究思路也需不断地讨论及修正。今后有可能引入系统生物学的研究技术用于腧穴特异性的基础研究,临床试验需要多中心、大样本、多水平的设计以得出更有说服力的证据。 Acupoints have such special characteristics as thinner epidermis, abundant nerve endings, dense receptors, blood vessels and lymph. Muscle spindle and connective tissue are possibly the fundamental configuration for special effects of acupoints as well as nerve system, there are more and more studies on the three-dimensional conformation of acupoints. The acupoints are frequently described as possessing distinct electrical properties, including high conductance, low impedance and resistance, high capacitance and voltage. New techniques and methods have emerged to observe the acupoints' electrical properties. Many studies reported the biochemical properties such as enrichment of Ca^2+ and reduction of Na^+ concentration. Results of many basic and clinical researches indicated that stimulating acupoints could produce the special therapeutic effect differing from other points. Using neuroimaging techniques to observe the brain's response to acupuncture demonstrates a promising method. Some results did not provide evidence to approve the specificities of acupoints. The conclusion variance of the study on acupoints' specificity requires to discuss the methodology, recognize meaning of comparative specificity of acupoints, and modify the design of the research. The systematic biological technique may be introduced in future, and clinical trial requires multi-center, large sample, and multi-level design.
