手术治疗20例胫骨Pilon骨折临床观察

摘要 目的:探讨胫骨Pilon骨折手术治疗的有效方法以及影响疗效的相关因素。方法:Pilon骨折20例,其中PilonⅠ型骨折4例,Ⅱ型骨折10例,Ⅲ型骨折6例;闭合性Pilon骨折6例,开放性GustiloⅠ型Pilon骨折4例,GustiloⅡ型骨折7例,GustiloⅢ型骨折3例;急诊手术复位胫骨远端前外侧旋转解剖型钢板(MAY钢板)固定9例,有限切开复位、支架外固定3例,石膏外固定4例,延期手术复位内固定4例。结果:按Mazur标准,优10例,良5例,可3例,差2例。结论:骨折类型、软组织损伤程度、治疗方式、胫骨关节面的复位质量和手术时机的选择、腓骨骨折的复位和固定、移植骨的填充及钢板支撑固定等是Pilon骨折治疗成功的关键因素。
