摘要
目的分析早期安氏Ⅱ类边缘型牙列拥挤非拔牙矫治的牙面形态变化,并探讨其建立正常关系的机制。方法选择12例安氏Ⅱ类边缘型牙列拥挤的病例,采用活动矫治器推磨牙向远中配合固定矫治技术进行治疗,在治疗前、活动矫治器治疗后和固定矫治治疗后拍摄头侧位片,对三个阶段的牙面位置进行测量和分析。结果活动矫治器治疗后上颌第一磨牙整体远中移动并轻度远中倾斜,上切牙轻度唇倾,上齿槽座点前移;固定矫治后,上颌第一磨牙位置较活动矫治器治疗后向近中移动。结论采用活动矫治器远中移动上颌磨牙,有利于中性的建立;Ⅰ类关系的建立可能是磨牙远中移动和颌骨差别生长的综合结果。
Objective To investigate morphologic changes of Angle class Ⅱ moderate crowding patients treated orthodontically without extraction and to analyze the mechanism. Methods Twelve patients with Angle class Ⅱ moderate crowding were recruited. The lateral cephalometric radiographs were evaluated at every stage before treatment, after molar distalization, and after treatment in order that the changes in skeletal, dental and soft tissues were determined. Results The distal movement and the distal angulation of the maxillary molars, mesial angulation of the maxillary incisors, and mesial movement of the maxilla were significant after molar distalization stage. Conclusion The upper molars' distalization can be achieved by removable appliance. The class Ⅰ molar relationship can be obtained by the differential growth of jaws and the differential movement of upper and lower molars.
