老年充血性心力衰竭并多脏器功能衰竭与氧自由基的关系及药物干预效果的研究 被引量:2

Relationship between Oxygen Free Radicals and Congestive Heart Failure with Multiple Organ Function Failure in Elderly as Well as the Effects of Drugs Intervention

摘要 目的研究银杏达莫治疗老年充血性心力衰竭并多脏器功能衰竭的效果及抗氧化自由基的作用。方法选择50例正常对照组及78例老年充血性心力衰竭并多脏器功能衰竭的患者,随机分为常规治疗组、银杏达莫组、复方丹参组。3组共治疗4周。对照组及3组患者治疗前后进行血清超氧化物岐化酶(SOD)、丙二醛(MDA)、过氧化脂质(LPO)、谷光甘肽过氧化物酶(GSH-PX)水平的测定。结果常规治疗组、银杏达莫组、复方丹参组治疗前SOD,GSH-PX水平明显低于正常对照组(P<0.01),MDA,LPO明显高于对照组(P<0.01);银杏达莫组、复方丹参组治疗后SOD,GSH-PX水平明显高于治疗前(P<0.01),MDA,LPO明显低于治疗前(P<0.01);常规治疗组治疗后各指标水平有所改变,但无统计学意义(P>0.05);银杏达莫组治疗后SOD,GSH-PX水平明显高于复方丹参组和常规治疗组(P<0.01),MDA,LPO明显低于复方丹参组和常规治疗组(P<0.01)。结论银杏达莫有明显的抗氧化作用,为阻断多脏器功能衰竭的发生发展、降低病死率提供了一条重要措施。 Objective To study the effects of Ginkgo Leaf Extract and Dipyridamole in treating congestive heart failure( CHF) with multiple organ function failure(MOF) in elderly, and its effect on anti-oxygen free radicals. Methods Choose fifty people as normal control group. Seventy-eight patients with CHF with MOF in elderly were randomly divided into three groups. Standard therapy group, diuretics and angiotensin-converting enzyme inhibitor ( ACEI ) , Ginkgo Leaf Extract and Dipyridamole group , Compound Danshen group. After four weeks,treatment, the changes of superoxide dismutase ( SOD ) , malondialdehyhe ( MDA ), lipid peroxidr ( LPO ) and glutathione peroxidase (GSH-PX) were detected in the three groups,patients. Results ①Before treatment the levels of SOD and GSH-PX in standard therapy group, Ginkgo Leaf Extract and Dipyridamole group and compound Danshen group were significantly lower compared with the normal control group( P 〈0. 01 ) ,while the levels of MDA and LPO were significantly higher( P 〈0. O1 ) ;②Compared with pre-treatment,after treatment the levels of SOD and GSH-PX in Ginkgo Leaf Extract and Dipyridamole group and compound Danshen group were significantly higher( P 〈0. 01 ) ,while the levels of MDA and LPO were significantly lower( P 〈0. 01 );③The changes of SOD,GSH-PX,MDA and LPO in the standard therapy group did not reach statistical significance( P 〉 O. 05) ;④After treatment, the levels of SOD and GSH-PX in Ginkgo Leaf Extract and Dipyridamole group were significantly higher compared with compound Danshen group and standard therapy group( P 〈 0. 01 ) ,while the levels of MDA and LPO were significantly lower( P 〈 0. 01) ;⑤In compound Danshen group, after treatment the levels of SOD and GSH-PX were significantly higher compared with standard therapy group { P 〈 0. 05 ), while the levels of MDA and LPO were significantly lower ( P 〈 0. 05 ). Conclusion Anti-oxidant action of Ginkgo Leaf Extract and Dipyridamole was significant. Ginkgo Leaf Extract and Dipyridamole plays a critical role in intercepting the occurrence and development of MOF and reducing the mortality.
