难治性癫痫患者外周血多药耐药基因表达的研究 被引量:1

摘要 目的研究人多药耐药基因(MDR1)、mRNA及其产物P-糖蛋白(P-gp)在外周血中的表达。方法用流式细胞仪定量检测难治性癫痫(RE)患者(A组)、癫痫控制良好患者(B组)和健康献血者(对照组)的人外周血P-gp表达,荧光定量聚合酶链反应技术检测其MDR1-mRNA表达。结果两种方法均显示,RE组MDR1-mRNA表达量较B组和对照组显著增高(P均<0.01);B组与对照组比较无统计学差异。A、B组及对照组P-gp阳性细胞表达率为0者分别为2、3、7例,而三组均有MDR1-mRNA表达,如表达量按>105为参考值,则RE组阳性率为100%。结论定量检测癫痫患者外周血MDR1-mRNA表达,可同步反映其脑组织MDR1表达水平,作为RE诊冶的外周指标;荧光定量PCR较流式细胞仪检测MDR1-mRNA表达的灵敏度及特异性高。 Objective To investigate the express of human multidrug resistance gene( MDR1 ) -mRNA and P-glycoprotein(P-gp) which was the product of MDRI-mRNA in peripheral blood. Methods The express levels of P-gp in peripheral blood of refractory epilepsy patients( A group), controlling well epilepsy patients( B group) and healthy subjects( control group) were detected by flow cytometry, and MDRI-mRNA express levels of them were detected by fluorescence quantify polymerase chain reaction (PCR). Results The two methods both showed that MDR1 express in A group was higher than that in B group and control group(P all 〈0.01 ) , and there was no difference in B group and control group. Flow cytometry showed that the patients of A, B and control group whose positive express rates of P-gp were zero were 2,3 and 7, separately, but fluorescencequantify PCR showed that MDR1-mRNA expressed in the three groups. If reference value of express quantity was 〉 10^5, the positive rate of MDRI-mRNA in A group was 100%. Conclusions Quantifing the expression levels of MDRI-mRNA in peripheral blood synchronously reflect the expression levels of MDR1 in brain tissue. It can be a peripheral guideline in making a diagnosis and giving treatment of refractory epilepsy. Comparing with flow cytometry, fluorescence quantify PCR has higher sensitivity and specificity.
