摘要
目的探讨胆囊切除术致胆囊床胆管损伤的部位、原因及处理。方法对15例胆囊切除术中胆囊床胆管损伤的病例进行回顾性分析。结果15例病例中，证实为Luschka胆管7例，胆囊肝管3例，右前叶肝管及其分支3例；其余2例仅发现胆瘘而未见损伤胆管。本组15例皆痊愈出院，无手术死亡。结论对胆囊床胆管解剖的认识对于减少胆囊床胆管损伤相当重要，对于胆囊床胆管的损伤应当尽量在术中发现并处理。
Objective To explore the causes and position for the bile duct injury in gallbladder bed and investigate its diagnosis and management. Methods The data of 15 cases of bile duct injury in the gallbladder bed after choleeystectomy were retrospectively analyzed. Results According to the operative records, 7 among the 15 cases had leakage from Luschka bile duct , 3 cases from the cholecystohepatic duct and 3 cases from the right iobuiar hepatic duct and its branches. The sources of the leakage were not identified in other 2 cases. All the 15 cases were cured. Conclusions During performance of cholecystectomy , surgeons should pay close attention to the bile duct in the gallbladder bed for its vulnerable position. The bile leakage in the gallbladder bed should be recognized and treated during the operation.
