摘要
目的观察针药并用对精液异常不育症的临床效应及针灸与不同药物并用之间的疗效差异。方法将37例精液异常不育症的患者分为针灸加中药组(25例)和针灸加西药组(12例),针灸治疗取穴关元、大赫(双侧)、三阴交(双侧),以低频电针配合药饼灸的方法,针灸加中药组同时服用中药二仙汤水泛丸,针灸加西药组则口服枸橼酸克罗米芬,治疗1个疗程后,对两组患者治疗前后临床症状、精液常规指标和血中Te、FSH、LH水平进行对比,通过统计学的方法观察治疗前后的变化情况及两组之间的疗效差异。结果针灸加中药组疗效显著高于针灸加西药组(P<0.05),针灸加中药组患者治疗前后精液常规在密度、1h后存活率、活力得分、正常形态率上均有改善(P<0.05),针灸加西药组除精液量治疗后减少(P<0.05),其余各项变化均无统计学差异。治疗前后激素水平对比针灸加西药组Te、LH均有显著升高(P<0.05),FSH也有升高,但差异无统计学意义。而针灸加中药组三项指标变化均不明显。结论针灸加中药治疗精液异常不育症有显著疗效。
Objective To investigate the clinical effect of combined use of acupuncture and medicine on infertility due to seminal abnormality and the differences in therapeutic effect between combinations of acumoxa and different medicines. Methods Thirty-seven patients with infertility due to seminal abnormality were randomly allocated to an acumoxa plus herbal medicine group （25 cases） and an acumoxa plus Western drug group （ 12 cases）. Points Guanyuan（ CV 4） , Dabe （ KI 12 ,bilateral） and Sanyinjiao （ SP 6 ,bilateral） were selected for acumoxa treatment. Low frequency electroacupuncture was combined with medicinal cake moxibustion. Meanwhile, the acumoxa plus herbal medicine group took Erxiantang Shuifan Wan and the acumoxa plus Western drug group, Clomiphene citrate. The pre - posttreatment clinical symptoms, routine seminal indices, and blood Te, FSH and LH levels were compared in the two groups after one course of treatment. The posttreatment changes and the difference in therapeutic effect between the two groups were evaluated by statistical analysis. Results The therapeutic effect was significantly better in the acumoxa plus herbal medicine group than in the acumoxa plus Western drug group （ P 〈 0.05 ）. Spermatozoal density, survival rate after one hour, vitality score and normal shape rate in a routine seminal examination all improved in the acumoxa plus herbal medicine group after treatment （ P 〈 0.05 ）. In the acumoxa plus Western drug group, there were no statistically significant differences in the indices except that the seminal volume decreased after treatment （P 〈 0.05 ）. A pre-posttreatment comparison of hormone levels showed that in the acumoxa plus Western drug group, both Te and LH increased significantly （P 〈 0.05 ） , and FSH also increased but had no statistically significant difference. In the acumoxa plus herbal medicine group, all the three indices did no change significantly. Conclusion Acumoxa plus Chinese herbal medicine has a marked therapeutic effect on infertility due to seminal abnormality.
