摘要
目的探讨老年人涉及关节面的肱骨远端粉碎性骨折的手术疗效。方法2003年4月～2004年8月,对16例平均年龄64.8岁的老年肱骨远端关节内粉碎骨折病例,采用肘关节后方尺骨鹰嘴截骨入路,直视下复位,外侧重建钢板、内侧1/3管型钢板内固定治疗,早期行功能康复锻炼。术后对骨折愈合情况、肘关节功能和并发症等进行随访评估。结果术后平均随访22个月(12～31个月),骨折全部愈合,平均愈合时间11.5周(9～15周)。术后肘关节活动度平均113(°95°～145°)。Cassebarm's评分优5例,良9例,可和差各1例,优良率87.5%。术后出现尺神经麻痹1例,尺骨鹰嘴截骨未愈1例,经处理后均痊愈。结论采用合适的内固定技术和术后早期功能锻炼的方法治疗老年肱骨远端粉碎性骨折临床疗效良好。
Objective To evaluate the results of the open reduction and internal fixation treatment ofintra-articular comminute fractures of distal humerus in the elderly. Methods From March 2003 to August 2004, 16 patients averaged 64.8years with intra-articular comminute fractures of distal humerus were treated with operatively fixation. Through a posterior approach and olecranon osteotomy, the fracture was reduced visibly. The internal fixation was achieved with reconstructive plate and 1/3 tubular plate placed on the posterolateral and medial sides of distal humerus respectively. The situation of bone healing, function of elbow and the compilations were evaluated after treatment. Results Average followup was 22 （12 - 31） months. All the fractures united with the union time of 11.5 （9 - 15） weeks. The average range of ulnohumeral motion was 113°（95°- 145°）. Five patients got excellent functional results, 9 cases got good results, 2 patients got fair and poor results respectively, according to Cassebarm＇s scoring system. Conclusion A proper technique of open reduction and internal fixation with two plates is very useful and effective in the treatment of intra-articular fractures of the distal humerus.
