膝关节镜联合有限切开内固定治疗胫骨平台骨折 被引量:5

摘要 目的:探讨膝关节镜结合有限切开内固定治疗胫骨平台骨折的疗效,为临床提供可选择的微创治疗方法。方法:共42例患者施行膝关节镜联合有限切开内固定治疗,男27例,女15例。胫骨平台骨折按Schatzker分类法进行分类:I型5例,II型15例,III型12例,Ⅳ型7例,Ⅴ型2例,Ⅵ型1例。40例得到随访,平均随访时间18个月,采用Rasmussen评分评价疗效。结果:25例患者(55%)存在关节内软组织损伤。膝关节功能依照Rasmussen评分法,优19例,良16例,可5例,本组总评分为平均(27±1)分。结论:该治疗方法可以避免关节囊切开,同时处理关节内损伤,明显减少术后并发症,有利于膝关节功能恢复。 Objective : To investigate the clinical results of arthroscope combined with minimally invasive internal fixation for the treatment of tibial plateau fractures. Methods: Forty-two patients with tibial plateau fractures were treated with arthroscopically assisted limited invasive internal fixation. There were 27 males and 15 females ,with an average age of 38 years. Tibial plateau fractures were classified according to Schatzker staging system. There were 5 patients with type-I, 15 patients with type- Ⅱ , 12 patients with type-Ⅲ , 7 patients with type- Ⅳ, 2 patients with type-V and 1 patient with type- Ⅵ. Forty patients were followed up,with an average of 18 months. Evaluations were performed according to Rasmussen functional score system. Results:Over half(55%)of the patients had concomitant intra-articular lesions. The mean Rasmussen functional score was 27±1. Nineteen patients got an excellent result, 16 good,and 5 fair. One patient suffered from joint stiffness (ROM:0 to 90 degrees )and another patient developed degenerative arthritis during the follow-up period. Conclusion:The treatment has advantages of avoiding open the articular capsule, dealing with intra-articular lesions at the same time, decreasing the postoperative complications and profiting the recovery of knee joint.
