抑制补体激活对脓毒症大鼠肺组织的作用研究 被引量:1

摘要 目的观察补体抑制剂眼镜蛇毒因子(CVF)抑制补体激活对脓毒症大鼠肺组织的作用。方法144只清洁级雄性SD大鼠(体质量250~300g)被随机分为A、B、C3组,每组48只。A组(假手术组)仅开腹翻动盲肠后关腹,B组(脓毒症组)采用盲肠结扎穿孔术(CLP)复制脓毒症大鼠模型,C组(CVF组)在CLP前24h按50μg/kg注射CVF以耗竭补体,而A组和B组则在术前24h给予等量生理盐水。术后2、4、8、12、16、24h6个时间点处死动物后大体观察各组大鼠肺脏及胸腔积液变化,并采集肺组织标本检测肺组织MPO水平,同时观察肺组织病理学及超微结构变化。结果B组可见肺组织实变区及胸腔积液情况随时间增长逐渐加重,C组明显减轻,A组没有相关表现。A组肺组织MPO水平术后一直处于较低水平,而B组和C组则迅速升高,除2h外,其余时间点均显著高于A组(P<0.05或P<0.01),但C组显著低于B组(P<0.05或P<0.01)。肺组织病理学及超微结构变化提示B组损伤最严重,C组明显减轻。结论补体抑制剂CVF可以明显减少中性粒细胞在肺内的聚集,并有效减轻脓毒症大鼠的肺损伤,显示出对肺组织良好的保护作用。 Objective To investigate the protective effects on lung tissues by inhibition of complement activation by cobra venom factor(CVF)in septic rats. Methods One hundred and forty four SD rats(clean, male, 250-300 g) were randomly divided into three groups(each n = 48) : group A(sham group) :abdomen was opened and closed later but without cecal ligation and puncture(CLP), group B (sepsis group) : sepsis was induced by CLP, group C (CVF pretreatment group) :sepsis was induced by CLP after pretreatment with CVF. At 24 hours before the operation,rats were given a mainline dose of either 50 μg/kg CVF in group C or an equal volume of saline solution in both groups A and B. Eight animals were killed at 2,4,8,12,16,24 hours points after operation. Changes of lung tissues and pleural effussion were observed, and lung tissues were harvested after the animal died to determine the levels of MPO and investigate the changes of pathology and ultrastructure. Results The changes of lung tissues and pleural effussion were severe in group B than other groups. The MPO levels of lung tissues in group A remained low in all time points. However,the levels in group B and group C rose quickly after operation and were higher than that of group A significantly except the 2 h point(P〈0.05 or P〈0.01). The levels in group C were lower than that of group B significantly(P〈0.05 or P〈0.01). The changes of pathology and ultrastructure showed that the lung injury were severe in group B than in other groups and lung injury in group C were lessened significantly compared to that of group B. Conclnsion CVF could reduce the aggregation of neutrophil in the lung tissues and lessen the lung injury in septic rats significantly, which showed the protective effects on lung tissues of septic rats.
