摘要
目的比较经皮穿刺扩张气管造口术和传统气管切开术在ICU患者中应用范围及利弊。方法60例ICU患者根据气管切开方式不同分为两组，即经皮穿刺扩张气管造口术（PDT）组和传统的气管切开术组，比较两组手术时间、术中出血量、切口大小及各种并发症的发生率。结果PDT组的手术时间明显短于传统的气管切开术组（P〈0．01），出血量明显少于传统气管切开术组（P〈0．01），手术切口长度也明显短于传统组（P〈0．01），两组主要并发症比较差异有显著性（P〈0．005）。结论PDT操作快捷，易于掌握，可在床边安全进行，可作为气管切开术的首选方法。
Objective To compare the clinical application of endotracheal of pereutaneous tracheostomy （PDT） with traditional tracheostomy. Methods Among the 60 patients, according to the operation way were divided into two groups, pereutaneous dilatational tracheotomy and traditional tracheostomy. The operation time, the amour of bleeding, the size of incision and the complication rates were compared. Results The operation time, bleeding, the size of incision in PDT were less than those in traditional tracheostomy group （ P 〈 0. 01 ）, the effect of traditional tracheostomy group was higher PDT guoup （ P 〈 0. 005 ） . Conclusion This study confirms that PDT is not only asimpler and quiker precedure than traditional tracheostomy but also can be performed in bedside which would eliminating therisk of patient transport.
