摘要
目的：为探讨Cyfra21－1在非小细胞肺癌（NSCLC）患者治疗前后含量变化并观察其与临床症状变化的相互关系,并藉此来评估治疗效果及预后。方法：用放射免疫法测定51例经手术病理确诊的NSCLC病人血Cyfra21－1含量。术前检测1次，术后分两组，手术根治组34例，术后1d，7d,1个月，3个月分别抽血测定，病情稳定后每3月1次；姑息切除化疗组17例，术后测定1次，每次化疗前后定期检测，两组测定值用统计学处理（T检验）。结果：手术根治组NSCLC术前Cyfra21－1平均值（7．769±5．915）ng／ml，术后（1．870±1．406）ng／ml（x±s），本前术后对照（t检验）有高度显著性意义（P＜0．01）。姑息性切除化疗组化疗前Cyfra21—1平均值（6．860±3.6.69）ng／m；（x±s）化疗后（3.134±1．957）ng／ml，化疗后Cyfra21－1水平高度显著性下降（P＜0.01）。两组治疗后都有显著性下降，但手术根治组下降幅度更大.结论：Cyfra21－1是NSCLC高度敏感性、特异性的肿瘤标志物，本组敏感性为94．1％，它的含量变化与临床症状变化是正相关，它不但可用来诊断NSCLC,而且完全可以单独应用于治疗监测，评估治疗效果及预后的判定。
The aim of this study was to monitor the quantity of scrum Cyfra 21 －1 before and after treatment in NonSmall Cell Lung Cancer (NSCLC) patients, and to observe the relationship between the serum Cyfra 21 － 1 leveland the clinical symptoms of NSCLC in order to evaluate the effectiveness of the treatment and prognosis. Radioimmunoassays of serum Cyfra 21 － 1 levels were performed in 51 patients with NSCLC diagnostlcally confirmed bypathological study. Radioimmunoassays were performed once preoperatively in each patient, and at varying timespostoperatively depending on if a radical operation or palliative surgery and chemotherapy were per formed. Radioim－mpnoassays performed postoperatively show that the Cyfra 21－ 1 levels decreased considerably in both groups,rather more in the radical operation group. This study indicates that Cyfra 2l － 1 is a highly sensitive and specifictumor marker of NSCLC. The sensitivity value was 94. 1 % in this study. The changes in the levels of serum Cyfra21 － 1 positively corrclated with the variation of clinical symptoms, so that not only can it be used to diagnoseNSCLC, but it can also be used to monitor treatment and assess prognosis of the NSCLC patient.
