摘要
１９９２年５月～１９９６年８月，行２２种腹腔镜手术５２２例，３例漏诊并存的恶性肿瘤（０．５７％）。３例平均延误时间为１．２７个月，平均年龄５９．７岁，分别为阑尾周围脓肿漏诊乙状结肠癌、胆囊结石漏诊胰头癌、胆囊积脓伴结石漏诊胃底癌，仅１例得到根治切除。另１例（０．１９％）２３岁女性，开腹阑尾切除术后出现肠梗阻，误为粘连性肠梗阻，腹腔镜手术中明确为横结肠癌。强调术者重视此问题并注意分析不典型的症状体征，术中仔细探查，主张术中超声检查。
From March 1992 to Aug.1996, We performed laparoscopic procedures in 522 cases for 22 different diseases.Concomitant malignancies were overlooked in 3 cases,including periappendicular abscess with sigmoid colon career in one,gallstone with pancreatic cancinoma in one,and carcinoma of the stomach together with gallbladder empyema and stone in one.The median diagnostic delay was1.27 months.Only one case underwent radical operation there after.Another female patient was found to suffer from carcinoma of the transverse colon by laparoscopy in an attempt of lysis at a tantative diagnosis of adhensive bowl obstruction after open surgery for her appendicitis. It is necessary to make a careful analysis of the atypical symptoms before performing operation,make careful exploration at laparoscopic operation and use laparoscopic ultrasonography.
