摘要
目的观察儿童少年期原发性高血压左室结构及心功能的变化,对高血压患儿靶器官异常进行评估。方法应用超声心动图对47例儿童少年期原发性高血压患儿和62名健康体查者进行检查,测量其心脏结构和功能改变。结果高血压组舒张末期室间隔厚度(IVST.D)、舒张末期左室后壁厚度(LVPWT.D)、舒张末期左室内径(LVED.D)、收缩末期左室内径(LVES.D)、左室质量(LVM)、左室质量指数(LVMI)较正常组显著增高(P均<0.01),射血分数(EF)、左室短轴缩短率(FS)较正常组降低(P<0.05)。舒张早期充盈峰速度(E峰)较正常组下降,但差异无统计学意义(P>0.05),舒张晚期充盈峰速度(A峰)上升(P<0.05),E/A显著下降(P<0.01)。11例患儿存在左室肥厚(LVH),出现心脏构型改变。结论在儿童少年期原发性高血压病中,作为病变累及的靶器官心脏存在结构和功能的异常改变。
Objectives To evaluate the changes of cardiac function and structural parameters in children and adolescents with essential hypertension (EH), and to assess the adverse effects on target organ. Methods Cardiac function and structural parameters were measured in 47 EH children with eehocardiography (EH group) and 62 healthy children were studied as control. Results Compared with control group, interventricular septum thickness in end diastole (IVST.D), posterior left ventricular wall thickness in end diastole (LVPWT.D), left ventricular diameter in end diastole (LVED.D), left ventricular diameter in end systole (LVES.D) , left ventricular mass (LVM), left ventricular mass index (LVMI) were all significantly higher (P 〈 0.01 ) ; while ejection fraction (EF), left ventricular fractional shortening (FS) were significantly lower in the EH group (P 〈 0.05). Descending E peak in end diastole was lower in the EH group, but with no statistical significance (P 〈 0.05). In the EH group, peak in end diastole was enhanced (P 〈 0.05), and E/A ratio was significantly lower (P 〈 0.01 ). Some of the patients were found to develop left ventricular hypertrophy. Conclusions In children with hypertension, the heart as a target organ had developed some degree of functional and structural changes(remodeling).
