The Value of Color Doppler Ultrasonography for Assessing Liver Transplantation by Examing Hemodynamic Changes of Hepatic Artery Perfusion

摘要 目的利用彩色多普勒超声检测移植肝肝动脉灌注参数的变化及其临床意义.方法利用彩色多普勒超声检测38例肝移植患者不同时期的肝动脉的峰值血流速度(PS)、阻力指数(RI)、灌注量(VF)、灌注指数(DPI)、加速度(HAAC)及加速时间(SAT).结果(1)术后15d内移植肝正常者肝动脉PS、VF、DPI均明显低于对照组(P<0.05);而RI明显高于对照组(P<0.05).(2)当肝动脉狭窄时狭窄远端的PS<40cms,RI<0.5,SAT>0.08s,HAAC<300cm/s.(3)发生排斥反应时肝动脉的PS、VF、DPI降低,RI增高,SAT和HAAC也会增高.结论彩色多普勒超声检测移植肝肝动脉灌注情况及肝动脉并发症的诊断具有重要意义,并可作为观察移植肝肝动脉灌注参数术后变化规律的重要手段. Objective To evaluate the value of color doppler uhrasonography for assessing liver transplantation and early diagnosis of complication by examing hemodynamic changes of hepatic artery perfusion. Methods Thirty - eight cases subject to orthotopic liver transplantation were examined by CDUS and following results were obtained, including peak systolic velocity (PS), resistance index (RI), volume flow (VF), Doppler perfusion index (DPI) of the hepatic artery, hepatic artery acceleration (HAAC), systolic acceleration time (SAT). Results (1) In the patients without obvious complication, PS, VF, DPI of HA decreased but RI rised as compared with those in control group (P〈 0.05) 15 days after operation. (2) When the hepatic artery stenosis occurred, PS 〈40 ends, RI 〈0.5, SAT 〉0.08 s, HAAC 〈300 cm/s. (3) While the livers of transplantation were rejected, PS, VF, DPI of HA decreased, RI increased, SAT increased, HAAC increased. Conclusion CDUS has an important value in the diagnosis of hepatic artery complications following liver transplantation.
