摘要
目的了解老年肝炎肝硬化患者血清总胆汁酸(TBA)的变化情况,分析其与肝功能变化的关系。方法采用日本岛津CL-7200全自动生化分析仪测定138例老年肝炎肝硬化患者(Child-Pugh A级46例,B级48例,C级44例)的血清总胆汁酸水平、肝功能、凝血功能和96例青年肝炎肝硬化患者、60例健康对照者的血清总胆汁酸水平。结果老年肝炎肝硬化患者各组血清TBA水平显著高于对照组(P<0.001),血清TBA水平随Child-Pugh分级而递增;老年肝炎肝硬化患者血清TBA水平与Alb呈负相关,与ALT、TbiL、PT呈正相关;死亡组血清TBA水平显著高于存活组(P<0.001)。64例老年肝炎肝硬化好转患者血清TBA水平治疗后比治疗前低(P<0.001)。老年肝炎肝硬化组和青年肝炎肝硬化组比较,血清TBA水平无显著性差异(P>0.05)。结论老年肝炎肝硬化患者血清TBA水平的监测,对了解肝细胞损害的程度和估计预后均有一定的意义。
Objective To estimate the changes of serum total bile acids (TBA) in elderly patients with posthepatitic cirrhosis, and the relation between that and variation of liver function. Methods Serum total bile Acid levels, liver function and hemostatic function in 138 elderly patients with posthepatitic cirrhosis and serum total bile acid levels in 96 youth patients with posthepatitic cirrhosis and 60 healthy controls were determined with RI - 1000TM automatic biochemical analyzer. Results The levels of serum total bile acids in elderly patients with posthepatitic cirrhosis were significantly higher than that in healthy control(P 〈0. 001 ). The levels of serum total bile acids increased progressively with the cirrhosis child - pugh classification. It also showed negative correlation with Alb, positive correlation with ALT, TbiL and PT in elderly patients with posthepatitic cirrhosis. Meanwhile, it was significantly higher in patients died than those in the survival group of the same disease(P 〈0. 001 ). 64 elderly patients with posthepatitic cirrhosis for the best after trial were lower than before trial ( P 〈 0. 001 ). The levels of serum total bile acids were not significantly different in elderly patients with posthepatitic cirrhosis and youth patients with posthepatitic cirrhosis (P 〉 0. 05 ). Conclusions Monitoring of serum total bile acids is helpful in evaluating the degree of hepatocellular damage and the prognosis in elderly patients with posthepatitic cirrhosis.
