无痛技术在门诊人工流产中的应用 被引量:1

摘要 目的比较异丙酚、异丙酚复合芬太尼、异丙酚复合氯胺酮和异丙酚复合芬太尼氯胺酮用于门诊人工流产技术的镇痛效果和安全性。方法选择120例ASAⅠ级,年龄21~46岁,无心脏疾病拟行人工流产的早孕患者,随机分为4组;异丙酚组(A组),异丙酚+芬太尼组(B组),异丙酚+氯胺酮组(C组),异丙酚+芬太尼+氯胺酮组(D组),每组各30例,对用药前、术中、术后收缩压、呼吸、心率、脉搏、血氧饱和度的变化及镇痛效果、苏醒时间、有无不良反应进行观察。结果A组患者术中收缩压、心率明显下降(P<0.01),脉搏血氧饱和度下降、呼吸减慢(P<0.05),肢体扭动5例;B组患者术中心率减慢、脉搏血氧饱和度下降(P<0.05),术后苏醒时间延迟2例;C组患者术中呼吸减慢(P<0.05),术后苏醒延迟4例、诉头痛2例:D组患者镇痛效果好,苏醒快,生命体征基本平稳,无不良反应。结论异丙酚、芬太尼和氯胺酮三药联合应用,镇痛效果好,安全性佳,是用于门诊人工流产术中较为安全有效、解除病人疼痛的一种理想药物配伍方法。 Objective To compare the analgesic effects and safeties of diprivan, diprivan composite with fentanyl, diprivan composite with ketamine and diprivan composite with fentanyl and ketamine applied to the indolence techniques. Methods Choosed 120 cases of early pregnant patients without pulmonary heart disease who are 21-46 years old, level ASA I and plan to take induced abortion. They were divided into four groups randonfly: Diprivan (group A), Diprivan + Fentanyl(group B), Diprivan + Ketamine(group C), Diprivan + Fentanyl + Ketamine(group D), 30 patients in each group. Observed the systolic pressure, breathe, heart rate, pulse and de- gree of blood oxygen saturation before medication, during and after operation. Also to view the analgesic effects, the time of regaining consciousness and side effects. Results Group A: During the operation, the systolic pressure and the heart rate of patients obviously decreased (P〈0.01), and the degree of blood oxygen saturation decreased and the breathe slowed (P〈0.05), 5 patients have body writhing; Group B: During the operation, the heart rate, pulse and degree of blood oxygen saturation of patients decreased (P〈0.05), 2 patients delayed the time of regaining consciousness after operation; Group C slowed down the breathe, 4 patients delayed the time of regaining consciousness and 2 patients had headache after operation. The patients' analgesic effect D Group of was good, the time of regaining consciousness was quick, the vital sign of patients are basically balanced and no side effects. Conclusion The combined usage of diprivan, fentanyl and ketamine can gain better analgesic effects and more safeties. It is an ideal dispensing method to apply to out-patient clinical induced abortion and release the pain of patients.
