DWI在单发性脑转移瘤、脑胶质瘤和脑脓肿鉴别诊断中的价值 被引量:18

The Value of DWI in Differentiation among Solitary Brain Metastatic Tumors,Gliomas and Brain Abscesses

摘要 目的探讨磁共振弥散加权成像(diffusion-weighted imaging,DWI)在单发性脑转移瘤、脑胶质瘤和脑脓肿鉴别诊断中的价值。方法32例单发性颅内占位病变患者(脑胶质瘤18例、脑转移瘤8例、脑脓肿6例)均行MRI检查,包括T1WI、T2WI、T1WI增强及DWI扫描。分别测定病变实体部分、瘤内囊性部分、近瘤周水肿区及相应对侧脑白质区的表观弥散系数(ADC)值并计算出相对ADC值(rADC值)。结果脑转移瘤组和脑胶质瘤组瘤内囊性部分的rADC值与脑脓肿组比较差异有统计学意义(P<0.05)。脑转移瘤组和脑脓肿组近瘤周水肿区的rADC值与脑胶质瘤组比较差异有统计学意义(P<0.05)。结论近瘤周水肿区、瘤内囊性部分rADC值的测定,对脑转移瘤、脑胶质瘤和脑脓肿间的鉴别诊断有重要意义。 Objective To explore the value of DWI(diffusion-weighted imaging)in differentiating among solitary brain metastatic tumors, gliomas and brain abscesses. Methods MRI, using conventional and DWI,was performed in 32 patients with solitary intracranial space-occupying lesions, in which there were 18 gliomas,8 brain metastatic tumors and 6 brain abscesses. The apparent diffusion coefficient (ADC)of solid portion of tumors, cystic components of tumors,peritumor edama and contra lateral normal brain were measured. And relative ADC(rADC value)was calculated. Results Comparing with the brain abscesses group,the rADC value of cystic compon- tions of the brain metastatic tumors group and the gliomas group was statistically significant(P〈 0. 05). Comparing with the gliomas group,the rADC value of peri-tumor edema of the brain metastatic tumors group and the brain abscesses group were statistically significant(P〈0.05). Conclusion The rADC value of peri-tumor edema and cystic compontions of tumors are valuable to differentiating among the brain metastatic tumors,gliomas and brain abscesses.
