脑出血患者氧化与抗氧化状态的研究 被引量:8

摘要 目的探讨脑出血患者氧化与抗氧化系统功能状况及其临床意义。方法检测133例脑出血患者和100例健康对照者血浆一氧化氮(P-NO)含量、血浆维生素C(P-VC)含量、血浆维生素E(P-VE)含量、血浆过氧化脂质(P-LPO)含量、血浆超氧化物歧化酶(P-SOD)活性、红细胞超氧化物歧化酶(E-SOD)活性、红细胞过氧化脂质(E-LPO)含量。结果与对照组比较,患者组的P-NO、P-LPO、E-LPO平均值均显著升高(P<0.001),P-VC、P-VE、P-SOD、E-SOD平均值均显著降低(P<0.001),逐步回归分析发现,患者临床神经功能缺失程度积分与P-NO、P-VC、E-LPO值相关最为密切,而颅内血肿量却与P-NO、P-VE、E-LPO值相关最为密切。结论脑出血患者体内氧化抗氧化平衡严重失调,且这一病理改变在脑出血后继发性脑损害的形成过程中起着重要的作用。 Objective To study the status of oxidation and antioxidation system and to explore their clinical implications in hemorrhagic stroke.Methods The levels of plasma nitric oxide(PNO),plasma vitamin C(PVC),plasma vitamin E(PVE),plasma superoxide dismutase(PSOD)activity,erythrocyte superoxide dismutase(ESOD)activity,plasma lipoperoxides(PLPO) and erythrocyte lipoperoxides (ELPO) were measured in 133 patients with cerebral hemorrhage and in 100 healthy control persons.Results Compared with those in normal persons,the average contents of PNO,PLPO and ELPO in patients were significantly increased,while the levels of PVC,PVE,PSOD,ESOD were markedly decreased (P<0.001). The stepwise regression analysis showed that the score of neurological deficit(y) was closely related to the levels of PNO(x1),PVC(x2),ELPO(x3)(y=9.5839+0.0169x1-0.0821x2+0.6267x3,r=0.5695,P<0.0001),and that the hemorrhagic volume(y) was related to PNO(x1),PVE(x2),ELPO(x3)(y=23.7584+0.0225x1-0.3758x2+0.9912x3, r=0.6689,P<0.0001). Conclusion The results indicated that the balance between the oxidation and antioxidation systems in patients with cerebral hemorrhage was markedly disturbed,which might play an important role not only in inducing the cerebral hemorrhage but also in giving rise to secondary brain injury.
