摘要
目的分析广东省一、二、三级妇幼保健系统对早产儿视网膜病(ROP)筛查工作的开展情况,探讨ROP的筛查、防治策略,规范和完善我省的筛查工作,以降低ROP的危害。方法通过信函、现场调查和电子邮件等方式对广东省境内新生儿重症监护室(NICU)的54家妇幼保健院(站、所)进行ROP的筛查情况调查,内容包括是否开展ROP筛查、氧浓度监测、近5年的筛查例数和阳性例数等。结果医院级别越高,筛查普及率和阳性率越高。共有13家妇幼保健院开展了对ROP的筛查,占调查例数的24·07%,经筛查的4304名新生儿中,ROP患儿90例,阳性率2·09%。一、二、三级妇幼保健院的筛查阳性率分别为0·0001%、0·5%、和4·5%,各级保健系统的筛查阳性率差异有统计学意义(P<0·01)。结论广东省各级妇幼保健系统对ROP筛查的开展情况不理想,筛查阳性率低且各级系统差异较大,应提高对此疾病的防治力度,培养专业筛查人员,提高对高危新生儿的筛查覆盖面和对此疾病的诊断水平。
Objective To analyze the current status of retinopathy of prematurity （ROP） screening in premature infants in primary, secondary and tertiary women and institutes in guangdong province; to study the policy of screening, prevention and achieve a standardized approach and to refine the screening of ROP. Methods children health care treatment of ROP; to The data about ROP screening were collected via mail, on site survey or e-mail from the 54 Women and children health care institutes with neonatal intensive care units （NICU） in guangdong province during the past 5 years. The collected information included the condition of screening and oxygen therapy monitoring, clinical ROP diagnosis and incidence. Results ROP screening and oxygen therapy monitoring was carried out better in higher level institutes. There were 13 Women and children health care institutes that provided ROP screening （24. 07% ）. Among the 4304 infants screened for ROP, there were 90 positive cases and ROP incidence was 2. 09%. The ROP incidence in primary, secondary and tertiary institutes was 0. 000 1%, 0. 5 % and 4. 5 % respectively, with statistical of ROP screening at different levels of Women not satisfactory, the positive rate of ROP scree significantce （P 〈 0. 01 ）. Conclusion The current status and children health care institute in Guangdong province is ning is low and unbalanced at different levels of hospitals. It is urgent to increase the efforts to train more professional personals for ROP screening, to increase the coverage of screening for high risk infants, to improve the level of diagnosis of ROP, to improve prevention and treatment of the disease.
