摘要
目的探讨眼前段光学相干断层扫描仪(anterior segment optical coherence tomography,AS—OCT)测量正常人眼前段生物学结构的临床应用价值。方法采用AS—OCT扫描仪测量正常人105例(105只眼)的中央角膜厚度(CCT)、前房深度(ACD)、晶状体厚度(LT)和前房横径(ACHD);比较性别间的差异,分析年龄与它们的关系。结果正常人105例(105只眼)的CCT、ACD、LT及ACHD的平均值分别为:(525.16±30.58)μm、(2.97±0.32)mm、(4.21±0.43)mm、(11.68±0.35)mm。女性ACD(2.87±0.33)mm比男性(3.07±0.29)mm浅(t=3.482,P=0.001);CCT[女:(525.32±31.21)μm,男:(525.00±30.23)μm,P=0.957]、LT[女:(4.26±0.42)mm,男:(4.16±0.43)mm,P=0.250]和ACHD[女:(11.64±0.32)mm,男:(11.73±0.38)mm,P=0.217]性别间差异无统计学意义。CCT(r=0.067,P=0.514)、ACHD(r=-0.137,P=0.179)与年龄无相关性;ACD(r=-0.570,P=0.000)与年龄呈负相关,LT(r=0.806,P=0.000)与年龄呈正相关。随年龄的增加,ACD每年变浅lOμm;LT每年增厚19μm。结论AS-OCT能无创、精确、定量地测量正常人CCT、ACD、LT及ACHD。女性的眼前段解剖结构比男性小,随着年龄的增加,ACD逐渐变浅,LT厚度逐渐增加。
Objective To evaluate the clinical value of anterior segment optical coherence tomography (AS=OCT) on measuring the biometric structures of anterior segments of eyes in normal subjects. Methods One hundred and five eyes of 105 normal subjects were enrolled, whose biometric structures of anterior segments were measured using AS-OCT. The measuring items included central cornea thickness (CCT), anterior chamber depth (ACD), lens thickness (LT) and anterior chamber horizontal diameter (ACHD). The differences of biometric structures of anterior segments between genders were compared. The effect of age on the biometric structures of anterior segments was analyzed using linear correlation and regression. Results The mean value of CCT, ACD,LT and ACHD were (525.16±30.58)1xm,(2.97±0.32)mm, (4.21±0.43) mm and(11.68 ±0.35)mm, respectively. The mean ACD of females was(2.87±0.33)nml, which was signifi- cantly shallower than that of males(3.07±0.29)mm (P=0.001). The CCT was(525.32±31.21)lxm in females and(525.00±30.23)1xm in males (P=0.957), LT was(4.26±0.42)mm in females and(4.16±0.43)mm in males (P=0.250), and ACHD was(ll.64±0.32)mm in females and(1.73±0.38)mm in males (P=0.217), which showed no significant difference between genders. Age had no significant effect on CCT (r=0.067, P=0. 514) and ACHD (r=--0.137, P=0.179). There were significant negative correlation between age and ACD (r=- 0.570, P=0.000), and positive relationship between age and LT (r=-0.806, P=0.000). The analysis showed decreases of ACD with age by 10(m per year, and the increase of LT with age by 19(m per year, respectively. Conclusion CCT, ACD, LT and ACHD of normal subjects can be measured quantitatively and accurately by AS-OCT. The biometric structures of anterior segments of females were shallower than that of males. With age,ACD become shallower, while LT increase in normal subjects.
