胰岛素泵不同大剂量输注模式对餐后血糖控制的比较 被引量:2

Postprandial glucose excursions following bolus insulin administration in subjects with type 1 diabetes

摘要 目的观察胰岛素泵的不同大剂量输注方式对餐后血糖控制的影响。方法15例1型糖尿病受试者,随机分组,先后分别采用4种不同的模式:试验餐为75g方便面,采用①一次输注全部大剂量胰岛素;②餐前10min一次输注70%大剂量胰岛素,60min后输注30%;③试验餐前10min一次输注70%大剂量胰岛素,30%通过餐前30min开始,持续150min的临时基础率;④试验餐前10min开始120min的双波大剂量,分别为70%和30%。血糖仪监测餐前30min及餐后每隔30min至餐后4h的血糖水平。结果在120、150和180min的双波及大剂量结合临时基础率的血糖变化值较低(P〈0.05),在90~180min的双波及大剂量结合临时基础率的血糖值较低(P〈0.05)。结论餐时增加基础率或使用双波大剂量方案的血糖控制更稳定。血糖控制更好。 Objective To determine the effect of different insulin bolus administration on postprandial blood glucose. Methods Fifteen type 1 diabetes subjects receiving continuous subcutaneous insulin infusion using insulin aspart consumed the same meal on four occasions 3 days apart. They received the same dose of bolus insulin on each of the four occasions randomly assigned and beginning 10 min prior to the meal as either a single bolus,two separate boluses with the first 70% beginning 10 min prior to the meal and the second 30% after 60 min,70% as a bolus beginning 10 min prior to the meal with 30% as temp basal lasting 150 min beginning 30 min prior to the meal or a dual-wave (70% as a bolus and 30% as a square-wave over 2 h). Blood glucose levels were measured 30 min before and every 30 min after meal, and then for 4 h later. Results Changes in blood glucose values were the lowest after 120,150 and 180 min ( P 〈 0.05 ) when the dual wave and bolus plus temp basal was administered. When the dual wave and bolus plus temp basal were used,the patients had significantly lower glucose levels after 90-180 min ( P 〈 0.05 ). Condusion The dual wave and bolus plus temp basal can control blood glucose and keep it more stable.
