摘要
目的从不同亚型房室结折返性心动过速电生理学特性及射频消融前后房室结快径前传不应期差异,探讨其电生理学机制。方法178例慢快型房室结折返性心动过速患者中按消融完成顺序取连续30例,与慢慢型11例和快慢型8例,比较AH跳跃、心动过速时HA、AVA等电生理参数差异,以及射频消融术前后房室结快径前传不应期改变。结果慢慢型患者AH跳跃明显小于慢快型及快慢型,HA、AVA介于后两型之间;慢快型及快慢型消融术后快径前传不应期显著缩短,慢慢型则有轻度延长趋势。结论慢慢型房室结折返性心动过速电生理特性明显不同于慢快型及快慢型,消融前传慢径后房室结快径前传不应期未相应缩短,支持其折返环中尚存在逆传慢径。
Objective To investigate the clcctrophysiological mechanism according to the clectrophysiological characteristics of different sub-forms of AVNRT( atrioventricular nodal reentrant tachycardia) and differences in refractory period of fast pathway before and after radlofrequency catheter ablation. Methods Electrophysiological parameters of AH jump, HA interval and AVA et al. were compared between consecutive 30. cases of 178 patients with slow/fast AVNRT, llcases with slow/slow AVNRT and 7cases with fast/slow AVNRT. The differences in refractory period of antegrade fast pathway before and after RFCA were also compared. Results AH jump of slow/ slow patients was apparently shorter than that of slow/fast and fast/slow patients. HA interval and AVA of slow/slow patients were between slow/fast and fast/slow. Refractory period of slow/fast and fast/slow after RFCA became shorter remarkably. However,refractory period of slow/slow was prone to elongate slightly. Conclusion Electrophysiological characteristics of slow/slow AVNRT is quite different from slow/fast and fast/slow. Antegrade slow pathway ablated,refractory period of antcgrade fast pathway of slow/slow are not shorten. It supports that there exists retrograde slow pathway in rcentrant circuit of slow/slow AVNRT.
