人巨细胞病毒感染与脑梗死患者血管病变关系及可能机制的探讨 被引量:4

Relationship between the cytomegalovirus infection and the vasculopathy of cerebral infarction and the possible mechanism

摘要 目的探讨人巨细胞病毒(HCMV)感染与脑梗死患者血管病变关系的可能机制。方法选择首发脑梗死患者101例,根据实验室检查结果分为HCMV阴性组(59例)、HCMV阳性组(42例),同期选择健康体检的HCMV抗体阴性者40例为对照组。检测颈动脉内膜中层厚度(IMT)、HCMV抗体、脂联素、白细胞介素6(IL-6)水平,比较3组IMT、脂联素、IL-6水平。结果HCMV阳性组患者的IMT与其他2组比较有统计学差异(P<0.01);脂联素较其他2组明显降低(P<0.01);IL-6较其他2组明显升高(P<0.01);脂联素与IL-6呈负相关(r=-0.455,P<0.001);IMT与IL-6呈正相关(r=0.339,P<0.001)。结论HCMV感染可能通过升高血清IL-6水平,降低脂联素水平,促进动脉粥样硬化形成、从而诱发脑梗死。 Objective To study the relationship between the cytomegalovirus infection (HCMV) and vasculopathy in patients with cerebral infarction. Methods 101 patients with first stroke attack were divided into two groups:patients without HCMV infection and patients with HCMV infection,meanwhile forty healthy people were selected as control group. The IMT of common carotid artery,levels of serum adiponectin and interleukin-6 were measured and compared between the 3 groups. Results Compared with HCMV negative group and control group,the IMT of the common carotid artery increased significantly ( P 〈 0. 01), the level of serum adiponectin decreased significantly( P 〈0.01) and the level of serum interleukin-6 increased significantly in the HCMV positive group( P 〈0.01). Correlation analysis showed that the level of serum adiponectin was negatively correlated with the level of serum interleukin-6 ( r = -0. 455, P 〈0. 001) and IMT was positively correlated with the level of serum interleukin-6 ( r =0. 339, P 〈0. 001). Conclusion The HCMV infection can increase the level of serum interleukin-6 which decreases the level of serum adiponectin so that atherosclerosis occurs at last and the cerebral infarction may be induced.
