摘要
目的对^131I治疗Graves病剂量的计算进行改进，探讨一种更为科学的^131I治疗剂量计算方法。方法Graves病并采用^131I治疗的患者187例，随机分为A、B两组，A组（93例）患者治疗剂量计算公式：^131I治疗剂量（Bq或μCi）=计划量（Bq或μCi／g甲状腺组织）×甲状腺质量（g）×100／甲状腺最高（或24h）摄^131I率（％）；B组（94例）患者按上述公式计算后，再结合患者的病情、症状等因素进行剂量调整。结果B组1次治愈率（94．7％）明显高于A组（86．0％），两组比较差异有统计学意义；B组早发甲状腺功能减退率（3．2％）和晚发甲状腺功能减退率（7．4％）均低于A组（12．9％和22．6％），两组比较差异均有统计学意义。结论本研究在以往计算^131I治疗剂量公式的基础上，充分考虑每一例患者的病情、个体差异，调整^131I治疗剂量，使患者的治疗更加科学、合理，治愈率明显增高，甲状腺功能减退发生率明显降低，减轻了患者的经济负担，取得了很好的经济效益和社会效益。
Objective To explore a more scientific method in the calculation of therapeutic dose of ^131I. Methods One hundred and eighty-sevenGraves disease patients with ^131I treatment were randomly divided into two groups （group A and group B）. ^131I dose treatment in group A （n = 93） was calculated according to the formula： ^131I treatment doses （ Bq or μCi） = planned doses （Bq or μCi/g thyroid）Хthyroid weight （g）Х100/tbe thyroid highesl iodine uptake rate or 24 hours iodine uptake rate （%）. On the basis of the calculation formula of ^131I dose treatment, dosage adjustment was considered with individual differences, patient＇s condition and other factors in group B （ n = 94）. Results The cure rate of a single dose of ^131I was significantly increased in group B. There was significant difference between group A and group B. The incidences of early onset and late onset hypothyroidism were markedly decreased in group B. There was significant difference between group A and group B. Conclusions On the basis of the previous calculation formula of ^131I dose treatment,dosage adjustment is fully considered with individual differences,patient＇s condition and other factors in this study. This makes the treatment more scientific and reasonable. The cure rate of hyperthyroidism is obviously increased,moreover the oecurrence rate of hypothyroidism is obviously decreased. It can reduce the economic burden of patients,and furthermore it will acquire good economic results and society benefit. This method is worth popularizing.
