摘要
目的探讨腹腔镜多脏器联合手术的临床应用价值。方法回顾分析237例腹腔镜多脏器联合手术的临床资料,其中双侧精索静脉曲张高位结扎术28例,腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)联合肝囊肿开窗术15例,LC联合胆总管探查术23例,LC联合肝血管瘤切除术12例,LC联合阑尾切除56例,LC联合消化性溃疡穿孔修补术20例,腹腔镜消化性溃疡穿孔修补联合腹腔镜阑尾切除术(laparoscopic appendectomy,LA)24例,LC联合精索静脉曲张高位结扎术20例,LC联合附件手术8例,LA联合附件手术31例。结果237例腹腔镜多脏器联合手术2例中转,其余235例均获成功;3例术后出现并发症,1例术后胆漏,1例术后早期炎性肠梗阻,1例术后右下腹炎性包块,均保守治疗痊愈出院。结论腹腔镜多脏器联合手术能够安全、有效、经济地处理多种腹部疾病、使患者能够充分享受微创外科手术的优越性,在基层医院也具有临床应用价值。
[Objective] To study the clinical value of laparoscopic multi-organs surgery. [Methods] The data of 237 cases, who had laparoscopic multi-organs surgery were retrospective analyzed, including 28 cases with high ligation of double spermatic cord vein with laparoscopic, 15 cases with laparoscopic cholecystectomy (LC) with fenestration of hepatic cyst, 23 cases LC with choledochal exploration, 12 cases LC with laparoscopic resection for hepatic hemangioma, 56 cases LC with laparoscopic appendectomy (LA), 20 cases LC with repair operation of perforation of digestive ulcer, 24 cases repair operation with laparoscopic of perforation of digestive ulcer with LA, 20 cases LC with high ligation of spermatic cord vein, 8 cases LC with adnexal operation, and 31 cases LA with adnexal operation. [Results] laparoscopic multi-organs surgery were successfully performed on 235 patients, and two patients were converted to laparotomy. Post-operative complications occurred in three cases, including one case of bile fisrule, one case of early post-operative inflammatory ileus, and one case of right underbelly inflammatory mass. The patients healed entirely with conservative therapy. [Condusion] Laparoscopic multi-organs surgery canadequately present the advantages of microsurgery by safe, effective and economical management of various abdominal diseases. It shows clinical application value in basic hospital management and should be promoted.
