摘要
目的分析和总结颈髓损伤后相关并发症的特点及其监护和治疗措施。方法对重症监护病房(ICU)的颈髓损伤并截瘫患者进行动态监测,观察并记录所发生的并发症,将相应处理措施进行分析和总结。结果共观察25例颈髓损伤患者,其并发症主要包括低钠血症(100%)、低血压(52%)、心动过缓(48%)、呼吸衰竭(64%)、肺部感染(32%)、泌尿系感染(32%)、体温调节异常(32%)、消化机能减退(64%)、褥疮(12%)等。经过相应处理后,25例中21例并发症治愈或好转,转专科继续康复治疗,4例自动出院。结论颈髓损伤并发症多,发生率高,其中一些并发症较危重。ICU的监护治疗对颈髓损伤并发症的防治具有重要作用。
Objective To analyze and summarize characteristics of complications and its treatments after cervical spinal cord injury（CSCI） in intensive care unit（ICU）. Methods When the patiens with CSCI were admitted into ICU ,their vital signs were monitored consecutively. The incidence of complications was recorded and the treatments related to those complications were analyzed and summarized respectively. Results Twenty-five cases were involved in this research. The complications mainly included hyponatremia（ 100% ） ,hypotension（ 52% ） ,bradycardia（ 48% ）, respiratory faihure（ 64% ）, pulmonary infection（ 32% ）, urinary system infection（ 32% ）, abnormal thermoregulation（ 32% ） ,digestive dysfunction（ 64% ） ,bed sore（ 12% ）. All those complications were treated properly. Twenty-one cases were discharged from ICU after the complications improved or recovered. Meanwhile ,4 cases were discharged of their own accord for some reasons. Conclusion There is a high incidence of complications after CSCI and some complications are serious. ICU can provide proper treament and plays an important role in the course of CSCI.
