急性冠脉综合征患者血浆TF水平的变化及其临床意义

摘要 目的观察急性冠脉综合征(ACS)患者血浆组织因子(TF)的水平及其临床意义。方法40例临床确诊的ACS患者,其中急性心肌梗死组(AMI组)20例、不稳定型心绞痛组(UAP组)20例。稳定型心绞痛(SAP组)20例和正常健康人13例作为对照组。采用ELISA法测定血浆TF水平。结果AMI组和UAP组血浆中TF水平明显高于SAP组和正常对照组,差异有统计学意义(P<0.05)。AMI组血浆TF水平明显高于UAP组(P<0.05),SAP组和正常对照组相比差异无统计学意义(P>0.05)。结论ACS患者存在异常激活的高凝状态,TF触发的外源性凝血途径在冠脉内血栓形成上发挥重要作用。 Objective To observe the changes of plasma tissue factor (TF) in patients with Acute Coronary Syndrome (ACS). Methods 20 patients with acute myocardial infarction (AMI) and 20 patients with unstable angina pectoris (UAP) were studied, 20 stable angina pectoris (SAP) and 13 normal healthy volunteers served as control. Levels of TF were measured by ELISA. Results Plasma TF levels in AMI and UAP group were significantly higher than that in SAP and the healthy control groups ( P 〈 0.05 ) ; there were significant changes of plasma TF compared with those of patients with UAP ( P 〈 0.05 ) ; there were no statistical differences of TF in patients with SAP compared with those of healthy control groups ( P 〉 0.05). Conclusion There is an abnormally activated hyper - coagulation state in patients with acute coronary syndrome. TF is recognized as the initial trigger of the extrinsic coagulation pathway which plays a critical role in the pathogenesis of coronary thrombosis.
