摘要
按背驮式原位肝移植术要求,对17例成人肝和肝短静脉进行解剖观察与测量。结果显示:左、中、右肝静脉主干长为22.8±8.8mm,50.98±23.94mm,22.8±9.5mm;管径分别为10.74±2.86mm,9.5±3.75mm,15.6±4.05mm。右肝静脉除1例外,都以单独1支形式注入下腔静脉。中肝静脉和左肝静脉分三种形式注入下腔静脉:Ⅰ型4例,占23.5％,Ⅱ型12例,占70.6>6％;Ⅲ型1例,占5.9％。它们不在同一个平面注入下腔静脉。右、中肝静脉之间距离7～23mm,平均15.6mm。文章结合解剖学发现着重讨论了病肝切除和肝静脉成形术中的有关问题。
Topographic study of hepatic veins and small hepatic veins were made in 17 adultcadavers for piggyback operation in liver transplantation. The majority of small hepatic veins were found in middle and low segment of inferior vena cava (IVC) behind liver. The minority of the small veing were found in the high sigment in all cases ,but tow of 17 cases had a big accessory middle hepatic vein and one of the seventeen cases that right hepatic vein divided four branchs into IVC in the area. The length of left,middle and right hepatic vein trunk was 22. 8±8. 80mm,50. 83±23. 94mm and 22. 8±9. 50mm and the diameter was 10. 74±2. 86mm,9. 50±3. 75mm and 15. 6± 4. 05mm respectively. The right hepatic vein in all cases,except one flowed into IVC with one trunk. The middle and left hepatic vein flwed into IVC with one trunk, the middle and left hepatic vein flowed into IVC with tree ways:Ⅰ Type four cases(23. 5% ; Ⅱ Type 12 cases(70. 6%);Ⅲ Type one (5. 9%). The three hepatic veins-Flowed into IVC with nosame plane. The distance between right and middle hepatic vein was 7-23mm,average 15. 6mm. The paper discusses mainly matter concerning with removel of disease liver and fashion an orifice with the hepatic veins for the outflow anastomosis of the homograft suprahepatic ve-na cava according to the topography of hepatic veins.
