摘要
为迎接慢性病防治的挑战,我国进行了社区卫生服务改革。但由于社区卫生存在的诸多问题,使慢性病防治工作依然形势严峻。健康管理作为新的卫生服务理念,对于慢性病防治和社区卫生服务都有重要意义。健康管理与社区卫生服务具有互相促进、互相补充的关系,共同服务于慢性病防治工作,为健康管理和社区卫生服务提供了整合的平台。要在尊重我国国情、整合现有社区卫生服务资源的基础上,探索社区慢性病健康管理服务模式。
In order to meet the challenge of chronic disease prevention and treatment,community-based health reform has been introduced,but for those existing problems,the situation of chronic-disease prevention is still serious.As the innovative method of health-delivery reform,health management plays an important role in chronic disease prevention and treatment as well as community-based health delivery.Health management and community-based health are mutually promoted and complementany,which serves chronic-disease prevention and provides the platform for the integration.So community health management model should be explored,which is based on China characteristic and integrating existent community health delivery resources.
