摘要
目的探讨在同一台电解质仪上用同一患者的全血和血浆测定钾、钠、氯的结果是否具有可比性。方法每天选取符合EP9-A文件中方法对比实验数据分布要求的肝素抗凝静脉血样本4~8例进行双份测定(测定中指定第1例样本测定的顺序,再按反向顺序测定第2次),一共分析6d,合计40例样本,记录检测结果,按照EP9-A文件的要求和临床接受性能评价进行统计及偏差评估。结果通过对两种血液样本测定结果的统计学分析,在EP9-A文件允许范围内删除钾和氯的一个离群值后得到钾的相关系数为0.991;钠的相关系数为0.993;氯的相关系数为0.994,测定结果的偏差在允许范围内。除钾和氯外,钠有超过CLIA′88允许范围1/2的SE。结论经过样本对比及偏差评估,使用同一样本的全血和血浆在同一台电解质仪上进行钾、钠、氯测定,其测定结果具有可比性,临床接受性能评价在一定范围内具有可比性。
Objective To explore comparability of potassium, sodium and chlorine with whole blood and plasma with the same electrolyte analyzer. Methods Four to eight cases of heparin antieoagulated venous blood samples in line with the NCCLS-EP9-A file were collected for duplicate determination (one time with whole blood, and another time with plasma) of potassium, sodium and chlorine. The detected results of total 40 cases of samples were recorded. Statistical analysis and bias evaluation were performed according to NCCLS-EP9-A file and clinical acceptability evaluation. Results According to the statistical analysis on detected results, one outlier from potassium data and another outlier from chlorine data were deleted in the range of the NCCLS-EP9-A file, then we got the correlation coefficient of potassium was 0. 991; of sodium was 0. 993, and of chlorine was 0. 994. The result bias was in the acceptable range. Sodium was an elevated SE exceeding 1/2 of the permissive range of CLIA'88 standard except for potassium and chlorine. Conclusion Sample comparison and bias estimation show that the results of potassium, sodium and chlorine with whole blood and plasma with the same electrolyte analyzer are comparable in the clinical acceptable range.
