摘要
目的:总结外伤性肠穿孔的诊治方法及经验。方法:对我院1997年1月～2005年l2月外伤性肠穿孔70例患者进行回顾性分析。结果:70例患者均进行剖腹探查,进行一期肠修补50例,肠部分切除肠吻合术16例,肠吻合外置4例。术后一期愈合64例,切口感染5例,死亡1例;未发生腹腔脓肿、肠瘘。结论:早期诊断、早期手术、正确的手术方式、有效胃肠减压、腹腔引流、术后早期应用抗生素、静脉营养药物,是影响肠穿孔治疗效果的主要因素。
Objective： To summarize the experience of diagnosis and treatment of traumatic intestinal perforation. Methods： 70 patients with traumatic intestinal perforation from Jan 1997 to Dec 2005 and clinical data were analyzed retrospectively in our hospital. Results： Of all 70 patients were treated with exploratory laparotomy. Managements included primary repair of intestines in 50 patients, part resection and anastomosis in 16 patients, intestinal anastomosis-exteriorization in 4 patients. Postoperative results revealed primary healing in 64 patients, incision infection in 5 patients, death in 1 paticnt： there was no postoperative intra-abdominal abscess and intestinal fistula. Conclusion： It is important for the treatment of traumatic intestinal perforation to make earlier diagnosis,prompt and proper surgical operation, effective gastrointestinal decompression and intrapcritoncal drainage, earlier anti-inflammation and nutrition.
